Role Description The Senior Behavioral Health Care Advocate (CLIP Liaison) is responsible for coordinating and managing care for members with complex behavioral health needs across the continuum of care. Role Description We are seeking an experienced Licensed Practical Nurse (LPN) to join our remote care management team. This role works closely with clinical staff to provide patient education, monitor patient conditions, and communicate changes in condition to the appropriate healthcare providers. The LPN serves as a key point of contact for patients and helps support care plan adherence in a telehealth environment.
