Role Description We are seeking an experienced Licensed Practical Nurse (LPN) to join our remote care management team. This role works closely with clinical staff to provide patient education, monitor patient conditions, and communicate changes in condition to the appropriate healthcare providers. The LPN serves as a key point of contact for patients and helps support care plan adherence in a telehealth environment. This position requires an active New York LPN license and the ability to quickly adapt to a fast-paced remote care setting. Training will be approximately one week before assuming responsibility for a patient panel. Key Responsibilities - Engage patients in one-on-one telephonic and virtual interactions to reinforce care plans and support health goals - Deliver patient-centered nursing support and education - Collect and update clinical information, including health history, medications, vital signs, and patient-reported symptoms - Monitor patient progress through scheduled follow-ups and review of remote monitoring data - Identify and communicate changes in patient condition to the appropriate clinical team members - Educate patients on medications, symptom management, preventive care, and early warning signs - Support patients in implementing care plans through guidance on healthy lifestyle habits and disease management - Document patient interactions accurately and in accordance with established clinical standards - Collaborate with RNs, providers, and care team members to support positive patient outcomes - Assist with coverage across programs as needed to support operational and patient care needs Qualifications - Active Licensed Practical Nurse (LPN) license in New York - Minimum 3 years of clinical nursing experience - Strong patient education and communication skills - Ability to work independently in a remote environment - Experience documenting clinical information accurately and thoroughly - Proficiency with electronic health records and telehealth technology - Strong organizational, critical thinking, and time management skills Preferred Qualifications - Multi-state LPN license - Additional licensure in Texas, Hawaii, or Connecticut - Experience with cardiac care, heart failure, or chronic disease management - Telehealth, remote patient monitoring, or care management experience - Bilingual English/Spanish What We're Looking For - Ability to learn quickly and work independently with minimal supervision - Strong clinical judgment and patient engagement skills - Excellent communication and documentation abilities - Adaptability in a fast-paced healthcare environment - Commitment to delivering high-quality, patient-centered care Compensation $25/hr Interested in Learning More?
