In this role, you’ll sit at the intersection of technology and strategy—guiding clients through transformational data and cloud system initiatives, leading technical workstreams, and contributing to high-impact Salesforce delivery efforts across Sales, Service, and related Clouds. What You'll Do: Lead Data-Driven Process Improvement & Consulting - Serve as the main point of contact for technical workstreams, engaging with client and internal stakeholders. - Facilitate data and process discovery sessions, identifying areas for optimization and solution design. - Apply techniques such as process mapping, measurement strategies, and data analysis to improve client operations. - Contribute thought leadership on industry best practices and scalable Salesforce and data solutions. • Take ownership of complex data workflows, build scalable analytics pipelines, and contribute to productizing insights using modern data tools such as SQL, dbt, Airflow, Snowflake, and Databricks • Analyze large datasets to identify trends, patterns, and insights about coding output to enhance productivity and quality, and turn those needs into actionable reporting • Provide real-time data insights to business on-demand through ad-hoc queries • Use your technical skills and experience to streamline ad-hoc analyses into reusable data products where possible • Collect, interpret, and aggregate data from multiple data sources for supporting risk adjustment medical record coding and quality processes • Design, develop, test, and deploy reporting to support risk adjustment business users’ needs • Automate reporting and analytics when appropriate to make more scalable across customers and deliverable to a broader set of stakeholders • Identify trends in the reporting and work to partner with the teams to improve productivity and quality. • Run various risk adjustment models for Medicare Advantage, Medicaid or ACA to forecast patient risk scores and return on investment based on historical data and project variables • Work closely with cross-functional teams, including clients, to understand business needs, and determine the right methodology for analysis and assumptions to provide data-driven insights into program performance and partnerships • Create clear and concise reports to communicate findings and insights to both technical and non-technical stakeholders • Stay abreast of industry trends, new technologies, and methodologies to enhance the team's analytical capabilities • Help guide, mentor and train other junior analysts in support of their direct manager

Position Title: Intake Coordinator Report to: Intake Supervisor Position Details: The intake coordinator is a self-motivated, task oriented and able to operate in a busy and fast paced environment. A Team player that has the willingness to learn new processes and tasks, in a customer service world. This position is responsible for working with the intake team and individually reviewing, processing and inputting pt referrals into our provided software. Customer service, communication and multi-tasking are a big part of this position, as you may be required to make phone calls for updates or request to patients or referral sources. Key responsibilities: - Receive incoming referrals for processing. - Gather all admissions paperwork and entering it digitally. - Verifying medical insurance coverage and benefits for each patient. - Processing each referral in its entirety. - Answering phone and email inquiries. - Addressing any patient and referral source questions complaints or concerns. - Performing clerical work, such as copying, and faxing as needed. - Preparing and maintaining patient charts. - Complying with the regulations and policies of the facility. - Managing portals with incoming referrals for potential patients. - Any other intake job assignments given. - Be able to multitask when processing referrals, answering phones and responding emails. Minimum requirements: The requirements listed below must be documented and/or observed. - High school degree or GED - At least one (1) year experience in a home health intake - Prior experience working in a fast-paced office environment. - Comfortable working with Microsoft Office products. - Reliable transportation. - Available to work overtime if required. - Any other jobs you are assigned.
