Position Title: Intake Coordinator Report to: Intake Supervisor Position Details: The intake coordinator is a self-motivated, task oriented and able to operate in a busy and fast paced environment. A Team player that has the willingness to learn new processes and tasks, in a customer service world. This position is responsible for working with the intake team and individually reviewing, processing and inputting pt referrals into our provided software. Customer service, communication and multi-tasking are a big part of this position, as you may be required to make phone calls for updates or request to patients or referral sources. Key responsibilities: - Receive incoming referrals for processing. - Gather all admissions paperwork and entering it digitally. - Verifying medical insurance coverage and benefits for each patient. - Processing each referral in its entirety. - Answering phone and email inquiries. - Addressing any patient and referral source questions complaints or concerns. - Performing clerical work, such as copying, and faxing as needed. - Preparing and maintaining patient charts. - Complying with the regulations and policies of the facility. - Managing portals with incoming referrals for potential patients. - Any other intake job assignments given. - Be able to multitask when processing referrals, answering phones and responding emails. Minimum requirements: The requirements listed below must be documented and/or observed. - High school degree or GED - At least one (1) year experience in a home health intake - Prior experience working in a fast-paced office environment. - Comfortable working with Microsoft Office products. - Reliable transportation. - Available to work overtime if required. - Any other jobs you are assigned.

The Healthcare Data Analyst is responsible for supporting Cozeva + Vatica’s analytics efforts in support of the Growth team and broader corporate initiatives. These services include, but are not limited to, development of rapid client-facing outputs in support of new sales and expansion of existing relationships, enable visibility to risk adjustment market metrics, support other organizational efforts leveraging analytics. The Healthcare Data Analyst will serve as a partner with the BI reporting team and the Data Engineering team, filling time-sensitive needs with rapid-response solutions and subsequently supporting migration to institutional tools when appropriate.   Responsibilities: - Gather, interpret, and employ internal and public data to support identification of clear actions to drive corporate growth. - Refresh, interpret, and distribute analytics to support regular meetings and work groups. - Support collaboration with health plan clients to analyze, design, and develop networks through ongoing evaluation of both current and potential network participants. - Contribute to design and development of data-driven client solutions. - Analyze data related to practice/provider participation, to identify opportunities to increase the number of participating providers (in existing practices) and new practices participating in Cozeva + Vatica’s programs. - Participates in the development of targeting for sales campaigns. - Payer Opportunity Projections: Analysis of potential payer networks in preparation of Provider network campaign loads. - Using Access/Power BI desktop/Alteryx, load and evaluate pre-sale data files from prospective payers. - Identify contracted organizations (Wave Zero) and net new organizations (Wave One) having sufficient opportunity, factoring any Payer carve-outs. - Complete geo mapping as needed. - Evaluate members overlap with other Payers as needed. - Prepare Campaign load data in format defined by Salesforce administrator, and initiate Campaign load with Salesforce administrator following final approval.
