• Assist with telemedicine and in-home CPAP and BiPAP setups with patients and their families • Start with one 8 hour shift per week utilizing telehealth and in-home services • Potential transition into a 3 12 hour full-time position depending on the territory's necessity • Maintain HIPAA/patient confidentiality • Provide exceptional care to patients and work with family members to ensure comfort and respect for the patient • Demonstrates a professional attitude at all times • Maintain a positive and professional image and demonstrate self-directed learning to meet professional development and/or license requirements.

Job Requirements

  • Certified or Registered Respiratory Therapist or Registered Nurse
  • 1+ years’ experience in home health experience preferred but not required
  • Must be able to navigate multiple browser windows and screens
  • Demonstrates knowledge of Respiratory Care Field and ability to act as a resource for peers and non-clinical staff
  • Must be highly organized and demonstrates ability to handle multiple tasks at a time
  • Must be willing to work remotely via telehealth, in office, or in home environments
  • Excellent communication skills, both written and oral also required.

This role will be highly specialized and require strong critical thinking and creative problem-solving skills along with an aptitude for collaboration and teamwork. You will: - Perform telephonic outreach to caregivers of attributed patients to enroll children in care, support seamless onboarding, and re-engage those who missed their initial onboarding visit or have not utilized services post-onboarding. - Handle inbound phone calls from caregivers, health plan service coordinators, and hospital discharge planners as part of the enrollment and re-engagement process. - Drive re-engagement and enrollment outcomes through high-volume outbound outreach, securing enrollments during live patient calls, completing efficient warm handoffs, and coordinating appropriate and timely scheduling, while consistently meeting performance expectations tracked through call productivity, conversion rates, and engagement metrics. - Conduct thorough research on both attributed and enrolled patients to better understand healthcare needs and utilization, uncover potential barriers to enrollment or re-engagement, and identify effective strategies to engage caregivers. - Leverage relationships with healthcare professionals to support re-engagement of enrolled patients, collaborating closely with Community Health Workers, Outreach Specialists, RNs, and other cross-functional team members. - Collaborate with Provider Engagement Specialists to conduct embedded outreach (e.g., on-site at PCP offices, residential facilities) when appropriate to support enrollment and engagement efforts. - Develop deep knowledge of the Supplemental Security Income (SSI) program and conduct targeted outreach to caregivers who may be eligible. Role Description As a Patient-Centered Medical Home (PCMH), we are committed to providing accessible, high-quality, team-based care. Reporting to the Chief Medical Officer, the Remote Primary Care Access Clinician serves as a vital clinical extension of the interdisciplinary care team, focusing primarily on virtual access and inbox management. By efficiently managing the in-basket and conducting telehealth visits, this role ensures continuity of care, improves patient outcomes, and supports the broader OCHS team-based care model. A typical day consists of approximately 50% telehealth visits with patients and 50% in-basket management and other virtual support. Essential Functions - Serve as a member of the primary care team, providing telehealth and virtual support to an in-person primary care team. - Engage in regular and ongoing communication regarding the health of the population managed. - Utilize EPIC and a range of tools to maximize communication with patients and team members. - Collaborate with the Chief Medical Officer, Senior Medical Director, and nursing staff regarding the daily schedule. - Demonstrate flexibility and willingness to adapt based on daily patient needs. - Keep accurate, complete, legible, and timely medical and other records and files as required by law, which shall become and remain the sole property of OCHS. - Participate in peer review and patient care audits for quality assurance and utilization review to maintain and improve patient standards of excellence. - Maintain knowledge of current developments in specialty to enhance professional expertise, to include keeping current Board Certification and complete CME as needed to maintain licensure. - Maintain a productive and professional working relationship with other clinicians and healthcare professionals within and out OCHS. - Engage respectfully with OCHS administrative and support staff. - Perform care in accordance with all applicable state and federal laws, professional standards, and OCHS policies and procedures. - Other related duties and projects as assigned.
