Yes Essential Functions Provides MGB with a wide bed/service search following the established triage procedure. -Interviews the patient, family member or referring professional to obtain clinical and demographic data to determine need for evaluation and/ or admission: following established admission procedure. -Continually reassess census and bed availability. -Completes financial screening according to established procedure. -Enters patient information and ADT transactions into the Epic EHR system completely and accurately. -Completes admitting paperwork. -Interacts with departmental hospital staffs in a professional and cooperative manner. -Maintains confidentiality of information of the department and contributes to the effective operation of the department. The Hospice Admissions Coordinator processes patient referrals, coordinates the admission of patients, and enters patient admission data after all requirements of hospice have been met. This role will provide remote support to our programs serving R12 Hospice: Maine, Connecticut, Virginia, New Hampshire, Massachusetts, and New Jersey. The schedule will be Fridays: 4pm - 8pm, and Saturday and Sunday 8am - 6pm Eastern Standard Time.
