Role Description Chronic Care Staffing is looking for a dedicated Enrollment Coordinator to serve as the bridge between patients and our chronic care programs. In this role, you will be the first point of contact for patients, helping them understand how our Chronic Care Management (CCM) and Remote Patient Monitoring (RPM) services can improve their health outcomes. This is a vital position that directly impacts patient wellness while supporting our clinical teams. Daily Responsibilities - Conducting professional, compassionate outreach to patients via phone to introduce CCM and RPM programs. - Evaluating patient eligibility and clearly explaining program benefits and expectations. - Accurately documenting verbal consent and patient interactions in the EMR, strictly following CMS and HIPAA guidelines. - Collaborating with internal care teams to ensure smooth hand-offs and consistent patient support. - Maintaining high standards of documentation to ensure accuracy for care planning and billing. Long-term Success Over time, you will become a subject matter expert in our enrollment workflows, contributing to higher program participation rates and improved patient engagement metrics. You will be expected to manage your daily queue independently, ensuring that every patient interaction is helpful, informative, and compliant. Qualifications - Active, nationally recognized Certified Medical Assistant (CMA) or Registered Medical Assistant (RMA) certification. - MUST live in either MST or PST to be considered. - Bilingual fluency in English and Spanish. - Current BLS certification. - Direct, hands-on experience working with CCM and/or RPM programs. - Strong proficiency in Google Suite and electronic medical record (EMR) systems. - Ability to work independently in a secure, HIPAA-compliant home office. - Excellent verbal and written communication skills with the ability to explain complex information simply. Nice-to-Haves - In-depth knowledge of CMS regulations and documentation requirements for chronic care. - Previous experience in care coordination or transitional care. - Familiarity with clinical quality measures and reporting.
