• Coordinate and process medication refills, including data entry and claim adjudication • Place outbound refill calls and assist patients through the refill request • Respond to and resolve inbound patient and provider inquiries • Clarify prescription orders with pharmacists or clinical staff • Maintain and update patient and prescription data in software systems • Spot trends, recommend improvements, and support process-improvement initiatives • Partner with cross-functional teams to deliver a seamless patient experience • Maintain confidentiality and uphold high-quality service standards
Job Requirements
- 3+ years of pharmacy technician experience required: (refills, claims adjudication, clinical collaboration, or specialty pharmacy).
- Must have active Washington OR Oregon Pharmacy Technician license.
- National certification (CPhT or equivalent) required.

Role Description The Pharmacist, PR is responsible for developing, implementing, and managing programs to ensure that medications prescribed for kidney disease and its related comorbidities are evidence-based and cost-effective. The role involves interacting with physicians, advanced care practitioners, nurses, and other clinicians to provide guidance about medication stewardship with a focus on kidney disease and comorbidities prevalent in this population. The clinical pharmacist will guide Healthmap’s comprehensive pharmacy strategy and work collaboratively across Healthmap departments to bring value to all pharmacy related initiatives and clinical programs. Responsibilities - Review medication regimens and make recommendations on how regimens might be adjusted based on protocols and current guidelines. - Create educational material for providers, members, and staff regarding new drugs, new indications, guideline updates, and/or counter-detailing of industry advertisement. - Participate in the development of treatment algorithms, guidelines, policies and procedures as well as other kidney disease management efforts. - Evaluate medications that are high-cost, high-risk or high-volume using population health framework. - Optimize disease management for members. - Enhance processes related to medication reconciliation, medication adherence, and avoidable polypharmacy. - Conduct drug use evaluations or disease-based audits (e.g., review prescribing against predetermined criteria). - Collaborate with other care providers in the effective sharing of practice expertise. - Communicate clinical information accurately and comprehensively to other providers of patient care. - Adhere to policies, procedures and state and federal regulations to ensure compliance and confidentiality of Protected health information (PHI). - Participate in Compliance, HIPPA and other important trainings that align to Healthmap’s HR Security Policies. - Perform other duties as assigned. Qualifications - PharmD degree from an accredited School of Pharmacy with American Society of Health Systems Pharmacists (ASHP) Accredited Residency. - 3 years clinical pharmacy experience in working face to face with physicians and health care providers in hospital or ambulatory care. - Active Pharmacy License. - Renal patient pharmacy management and value-based care experience, Health Plan and/or Vendor experience preferred. - Understanding of Centers for Medicare and Medicaid Services (CMS) Star measures, Healthcare Effectiveness Data and Information Set (HEDIS) measures, MIPS and National Quality Forum measures. - Bilingual fluency in English and Spanish. - Must reside in Puerto Rico. • Interpret and fill drug orders accurately and timely • Ensures the appropriateness of all medication orders, new prescriptions or refills, in reference to the client’s diagnosis, weight/body surface area, allergies, and drug interactions as evidenced by a review of the active medication profile, provider notes, and available pertinent laboratory information. • Conduct/Review Drug Utilization Reviews to check for drug duplication, interactions or allergies. • Review non-formulary drug requests using predetermined criteria. • Review prior authorization requests using predetermined criteria. • Maintain current knowledge of drug therapies and drug reactions and interactions by reading professional literature, attending seminars and through professional affiliations. • Contact and confer with physicians regarding questions or irregularities with prescription practice whenever safety or established alternatives have not been utilized. • Monitor drug therapy for contraindications, interactions, allergies and appropriateness. • Documents / maintains initial patient assessments, reassessments and care management plans in the electronic patient management program, e.g., Asembia, CPR Plus. • Provide drug information to meet the needs of medical and nursing staff members through direct contact or in-service programs. • Provide patient information and counseling. • Participate as an active member and facilitator in process improvement teams. • Prepare provider newsletters or articles to keep providers updated. • Provide individual and group training, in-services and other consultations to unit staff and providers to optimize therapy. • Work with medical directors and staff in developing and carrying out various clinical initiatives. • Generate and review standard monthly and quarterly reports, as required. • Attend and participate in other programs, committees, meetings, and functions as required. • Assist in the development of clinical, utilization management, ad hoc and production reports and programs. • Assist in formulary, rebate analyses and pharmacoeconomic studies and initiatives. • Perform retrospective drug utilization review: identify drug, physician, and patient utilization trends; make interventions as appropriate; monitor and document outcomes. • Assist in the generation, maintenance, and distribution of formulary alternative lists, maintenance drug lists, and new drug to market lists. • Work cooperatively with other staff members to enhance the team concept. • Supervise pharmacy technicians in the routine performance of their duties. • Participate in the development and implementation of the Performance Improvement Program. • Assist in overseeing clinical procedures. • Complete facility audits as assigned. • Maintain regular attendance in accordance with established policies and procedures. • Perform other job-related duties as assigned. • Promptly report any allegations of impropriety to the Compliance Department. • Comply with VytlOne’s Ethical Business Conduct policy and VytlOne’s Compliance Program. • Remain free from exclusion under the OIG and SAMS Medicare/Medicaid lists. • Complete required training, as assigned, within the established timeframes. • Designated Person Only – Serve as the Designated Person by completing and maintaining all required training and certifications to ensure compliance with USP , USP , and all applicable company policies and procedures.
