Intake & Assessment Therapist

Role Description Intake Clinicians play a vital role within the company as they are the initial clinical contact. They are responsible for assessing and diagnosing both adolescents and adults with mental health concerns. Key responsibilities include: - Conducting thorough evaluations and gathering histories. - Formulating DSM diagnoses. - Assessing client's level of care needs and any risk factors. - Linking clients to appropriate services, either within the facility or through external referrals. This role requires strong clinical, diagnostic, and communication skills to ensure clients receive timely and appropriate mental health support. Qualifications - A master's degree in the Human Services field. - Full or Associate Licensure in state of practice; full licensure preferred. - 2+ years’ experience working in a Mental Health Treatment Facility. - Computer literate: Microsoft Office (Excel, Word, and PowerPoint) required. Requirements - Obtaining Informed Consent from clients. - Facilitating acceptance of client Informed Consent documents from patient portal. - Ensuring all documentation is completed prior to the start of the Intake Assessment. - Conducting assessments using PHQ and Columbia-Suicide Severity Rating Scale. - Gathering relevant clinical data for preliminary diagnosis. - Justifying diagnoses using DSMV criteria. - Maintaining accurate and concise documentation of client interactions. - Making clinically justified recommendations for treatment plans. - Facilitating linkage of clients to recommended services. - Identifying client’s current risk level and maintaining client safety through crisis intervention. - Attending monthly supervision and weekly Intake Clinical Meetings. - Maintaining Key Performance Indicator expectations. - Assisting and covering other staff as necessary.
