In this role, you will lead the planning, coordination, and delivery of complex technical initiatives across engineering teams. You will be responsible for driving delivery through structured planning, effective use of Jira, and strong stakeholder engagement. In addition, they will coordinate long term care services and supports for elderly and disabled individuals to help them maintain living independently in the community. Our goal is to assist individuals in applying for in-home care services and bridge access to such services through Presumptive Eligibility while providing case management through application process. About You: - You have experience and an in-depth understanding of the Case Management Program. - You have a desire to provide training and support to new and existing staff along with programmatic support to supervisors. - This person also may be expected to provide in-home care services to elderly and disabled adults, so they may live independently in the community. Position purpose: - The Case Manager Lead primary responsibilities include working with the Presumptive Eligibility program, training new staff and providing programmatic support to the supervisory team, working with clients to enroll in specialized programs in Case Management (Presumptive Eligibility), and being available to case managers for support and guidance. BASIC FUNCTION: - To perform responsible professional work in the planning, development, operation and delivery of case management and/or respite services, which includes assisting functionally impaired adults to obtain and effectively use support services, and enabling families and other caregivers to obtain temporary relief from their care-giving responsibilities. - Act as Fair Hearing Coordinator representing the Case Management Division in Administrative Hearings for Case Management clients.
