Care Coordinator
LIGHTHOUSE YOUTH SERVICES INC
Role Description Lighthouse Youth & Family Services is growing with our OhioRISE program! We are hiring Care Coordinators. We are looking for professionals that understand High-Fidelity Wrap Around practice while providing care coordination services to identified youth that will provide specific, measurable, and individualized services to each person served. Essential Functions: - Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity, Wrap Around model of care coordination. - Applies High Fidelity Wraparound principles, including coordinating and leading team meetings as needed. - Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary. - Ensure family support and stabilization during crises. - Provide and document the initial and ongoing Life Domain Assessment. - Maintain all service documentation requirements, evaluation outcome requirements and data. - Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan. - Utilize and monitor Flexible Funding and service coordination. - Obtain weekly reports from subcontracted providers. - Participate in after hour on-call response. Qualifications - High School Degree with three years of relevant experience; OR Associate/Bachelor’s degree with 2 years of relevant experience; OR Master’s Degree with 1 year of relevant experience. - Experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or behavioral health field providing community-based services to children and youth and their family or caregivers. - Experience in one or more the following areas of expertise: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development. - Excellent interpersonal and written communication skills. - Demonstrated computer proficiency.
