Role Description Do you love working with children and youth, making a difference in their lives, and watching them succeed? If so, the Outpatient Behavioral Health team at Lighthouse Youth & Family Services is growing fast and we need a QMHS Professional to provide case management services to help us carry out our mission and change the lives of children and youth. Essential Functions: - Provide community-based mental health case management services to youth, adolescents, and young adults. - Work closely with other professionals in providing treatment services in the areas of mental health, employment, education, and housing. - Have knowledge of community systems like dependency, foster care, adoption, and residential services. - Provide community-based services. - Develop treatment plans and maintain accurate and current documentation for client files. Qualifications - Bachelor’s Degree in Social Work or related field required. - Experience with youth, adolescents, and young adults with mental health issues and concerns preferred. - Experience with multi-system youth, adolescents, and young adults preferred. - Must have excellent written and verbal communication skills. - Must have a car and good driving record. - Bilingual and/or Sign Language proficiency a plus. Role Description Lighthouse Youth & Family Services is growing with our OhioRISE program! We are hiring an Intake Coordinator Supervisor. Responsible for overseeing assigned program operations and ensuring the success of the assigned service area program by ensuring employees perform their duties in alignment with the program’s philosophy and goals. Essential Functions: - Overseeing assigned program operations. - Monitors and ensures services within the assigned area meet community needs in a cost-effective manner and operate at maximum potential. - Assists in the management of the financial performance of the program and ensure all service level requirements are met. - Recruits, onboards, trains and develops program employees. - Responsible for the performance management of program employees which includes supervision and performance feedback. - Responsible for the program culture and addressing employee concerns with the support of HR. - Ensures timely completion of all notifications/paperwork which may include billing and attendance reporting. - Ensures compliance with licensure, accreditation, and certification requirements, including following CARF and other accreditation standards. Maintain positive relationships with all referring agencies. - Monitors and ensures the quality of services through ongoing evaluation, including maintaining high-fidelity in-service delivery and client treatment plans. - Engages in committees, presentations, and events that further the program’s mission and promote its services. - Assists the Director in creating and monitoring relevant statistical data, deliverables and compliance reports to ensure compliance standards of local, state and federal mandates. - Manages incident reports and collaborates with staff on appropriateness of documentation. - Develops and maintains relationships with community and advocacy groups affecting program and community services (e.g. county court probation offices, re-entry programs, ODYS legal services, etc.). - Participates in community activities that enhance the agency’s public awareness. - Develop new services and ensure that existing services meet the expectations outlined in grants and contracts, including required reporting, staffing, and financial management. - Ensure employees are trained in appropriate developmental/treatment strategies and ensure that treatment plans are followed. - Lead the creation of aftercare plans to address ongoing client needs post-service. - Complete ongoing skill and competency-based training, including ICC and MCC delivery, and wraparound training as provided by ODM-recognized entities. Qualifications - Bachelor’s degree or equivalent work experience. - At least two (2) years of experience in children’s behavioral health, child welfare, developmental disabilities, juvenile justice or behavioral health field providing community-based services to children and youth and their family or caregivers. - Demonstrated supervisory/leadership experience. - Experience in one or more the following areas of expertise: Family systems, community systems and resources, case management, child and family counseling or therapy, child protection, or child development. - Excellent interpersonal and written communication skills. - Demonstrated computer proficiency. - Bilingual preferred. We are looking for professionals that understand High-Fidelity Wrap Around practice while providing care coordination services to identified youth that will provide specific, measurable, and individualized services to each person served. Essential Functions: - Cultivate flexible, family-focused, community-based responsive services based on the High Fidelity, Wrap Around model of care coordination. - Applies High Fidelity Wraparound principles, including coordinating and leading team meetings as needed. - Coordinate and supervise implementation of the Plan of Care, including a Transition Plan and Crisis Plan, through service delivery with providers and community resources; update plan as necessary. - Ensure family support and stabilization during crises. - Provide and document the initial and ongoing Life Domain Assessment. - Maintain all service documentation requirements, evaluation outcome requirements and data. - Provide services in a timely manner and in accordance with Plan of Care and/or Crisis Plan. - Utilize and monitor Flexible Funding and service coordination. - Obtain weekly reports from subcontracted providers. - Participate in after hour on-call response.
