Overview The Case Manager will provide individual and group addiction counseling, perform clinical assessments and evaluations, develop and monitor individualized treatment plans, and ensure accurate documentation through record keeping and detailed progress notes. Required Skills and Abilities Bachelor’s degree in social work or related field with 1 year experience working with individuals who have a substance use disorder or equivalent combination of experience and education.
