Position Summary: Position Summary: The Community Nutritionist is responsible for providing a wide variety of nutrition assessment and education for agency clients, other consumers and members of our communities at large. This includes and is not limited to the providing of social determinate of health assessment for nutrition clients, agency consumers and referrals from councils on aging, billable Medical Nutrition Therapy, assessment, and education provided as part of contractual arrangements with payors, and community education events at housing sites, council on aging, other community-based organizations and, when necessary, in home. The Community Nutritionist will also perform activities in support of the agency's home-delivered meals program. The Community Nutritionist will provide clinical oversight to one or more staff people including the Community Nutritionist-Bilingual. Essential Functions: (Core duties or tasks that are fundamental and not marginal to job performance) - Provide SDOH and nutrition assessment, education, and associated feedback to clients of health care providers and systems with whom AgeSpan contracts, including but not limited to our Mass Health nutrition efforts; - Receive referrals and provide nutritional counseling to clients on special diets and/or those determined to be at a high nutritional risk; - Help to facilitate interagency communication and referrals for consumers with food insecurity, nutrition education and social determinate of health (SDOH) needs; - Conduct remote and/or field visits to assess an older adult's physical, social, emotional and environmental status to determine their eligibility and needs in their homes in accordance with contractual requirements. - Work with Nutrition department on the approval for medically tailored meals - Attend community health fairs to promote the agency - Prepare and present nutrition education programs at meal sites, councils on aging, housing sites and other community settings as needed; - Perform all implementation and data collection activities related to Medical Nutrition Therapy and other nutrition counseling and collaborate with Fiscal to ensure proper billing and collection; - Accurate and timely completion of all reports and documentation; - Clinically mentor the Community Nutritionist-Bilingual and interns - Perform other appropriate duties as assigned by Director, HLCE and Nutrition Director Qualifications: - Bachelor's degree - Registered Dietician or LDN licensed by the Commonwealth of MA required. - Serve-Safe Certified (can be provided post hire) - Strong written and verbal communications - Experience in nutrition education and counseling - Knowledge or experience in meeting nutritional health needs of an elder population - Ability to work independently and as a team member - Must have private transportation - Bi-lingual capacity a plus At AgeSpan, we carefully consider a wide range of factors when determining rate of pay.
