The Role As a Nurse Practitioner on our Virtual Care Management team, you'll deliver a member-first clinical experience across virtual visits, care planning, and care team support. You'll report to the Clinical Director and partner closely with our physician medical directors, care managers, and care navigators. This is a clinical role with a strong operational component. You'll provide direct patient care, guide the care team on day-to-day clinical decisions, and help build the workflows and processes that make our care model work. Responsibilities - Conduct virtual onboarding E/M visits, including history, assessment, documentation, orders/referrals, and follow-up planning - Provide clinical decision-making between visits; ensure timely care and appropriate escalation - Coordinate with member PCPs and specialists to support continuity of care - Review RPM data, alerts, and trends to identify members needing outreach or escalation; document clinical interpretation and route follow-ups appropriately - Escalate clinical concerns to the Clinical Director and/or medical directors per protocol - Partner with care managers to design, review, and approve individualized care plans aligned to evidence-based guidance and member goals - Provide day-to-day clinical guidance to care navigators and care managers; unblock escalations and mentor on documentation quality and escalation judgment - Help refine clinical workflows, templates, and protocols to improve safety, efficiency, and member experience - Identify gaps and propose improvements based on recurring needs, outcomes, and team feedback Requirements - Active, unrestricted Florida NP license in good standing - 2+ years of NP experience providing direct patient care (virtual care experience is a plus) - Strong E/M fundamentals: assessment, treatment planning, documentation, ordering/referrals - Comfort managing chronic and/or complex conditions; sound judgment on when higher-acuity evaluation is needed - Experience with telehealth, RPM, care management, or care navigation preferred - Excellent communication skills with a balance of empathy, clarity, and follow-through - Strong collaboration skills and good escalation judgment within a clinical team - High attention to detail across documentation and follow-up for multiple members - Comfortable in EHRs and digital tools; learns new systems quickly - Leadership or mentorship experience with clinical teams is a plus - Builder mindset: excited to improve workflows and playbooks as the organization grows - Ownership mentality and comfort operating in an early-stage environment where priorities evolve Compensation & Benefits We offer a competitive salary and benefits commensurate with experience, with the opportunity to make a meaningful impact in a mission-driven, fast-growing company. • Develop, implement and manage strategic fraud, waste and abuse activities by maintaining state and federal requirements and monitoring trends/schemes • Monitor business processes and systems to assure integrity and compliance in billing and claims payment • Lead teams of analysts to appropriately investigate all possible fraud, waste and abuse referrals • Develop customized fraud plans to meet contract and federal requirements • Develop educational materials to identify/validate waste activities as requested by the health plan and on an ad-hoc basis • Respond to RFP request and implement new policies per contractual obligation • Attend state/federal meetings as required by specific contracts • Prepare/present the FWA program to state/federal personnel upon request, specifically during readiness reviews, and immediately following the go live or upon state agency personnel changes • Review post-payment cases with appropriate parties to obtain refund • Prepare and distribute monthly and quarterly saving reports

• Oversee and monitor the performance of both internal and external cost recovery and cost containment initiatives according to targeted project metrics and forecasted recovery and cost avoidance goals for each Health Plan • Manage internal and external vendors cost avoidance and cost recovery initiatives • Partner with health plans to identify and contain cost through collaboration with national vendors for each recovery category • Develop annual budget and projected target for each national vendor and assist them with achieving projected annual savings • Evaluate and streamline existing processes and implement new processes to reduce costs • Present trends and cost containment opportunities, recommend action plans, and assist with the implementation of those plans • Assist health plans in understanding the rules, regulations and processes around cost recovery • Monitor other departments’ performance and processes and evaluate for cost recovery • Compile and Present reporting packages for Senior Management on current cost avoidance and cost recovery initiatives

• Defines and supports data governance strategy and goals. • Leads design, implementation, and continuous improvement of data governance and data architecture frameworks, policies, standards, and operating models. • Trains data stewards on their responsibilities, including standards compliance and supports them in completion of the work needed. • Evangelizes data governance strategy and requirements with technical and business audiences. • Evaluates and implements data governance operating models, tools and technologies. • Uses working knowledge of data query tools used to access existing data stores. • Review data models for compliance with data architecture standards – coach teams to compliance, when necessary. • Updates job knowledge by participating in educational opportunities. • Provides mentorship and guidance to junior analysts and data stewards.
