Home Infusion RN – Independent Contractor

• Conduct in-home infusion therapy visits with patients according to the plan of care. • Prepare clinical/nursing notes and document patient interventions in both clinical and non-clinical records consistently. • Act as a clinical resource, working collaboratively with other pharmacy departments. • Maintain knowledge of drug side effects and interactions. • Offer telephone and in-person patient education on therapy protocols, drug side effects, and home infusion procedures. • Ensure patients understand their therapy plans, medications, and the importance of adherence. • During initial and ongoing consultations, serve as a patient liaison between the patient, their family, and the pharmacy to ensure a seamless therapy process. • Perform comprehensive, initial, and periodic head-to-toe assessments for patients and initiate preventative and rehabilitative care. • Use health assessment data, and input from the agency team, the physician, the patient, and the family to determine patient needs. • Manage patient and family expectations regarding services, outcomes, and discharge goals. • Establish primary and secondary diagnoses based on patient assessment and home health care focus. • Develop care plans with skilled interventions, medical supplies, and necessary services to meet goals. • Regularly evaluate patient progress, in collaboration with team members, and revise care plans as needed. • Perform other duties as assigned.
Job Requirements
- Graduate of an accredited nursing school with a current RN state license.
- 2+ years of experience in home infusion and infusion centers.
- 2+ Experience with IV Immunoglobulins and IV Biologics.
- CRNI certification(Certified Registered Nurse Infusion) is preferred.
- Experience with peripheral IV starts, infusion therapy, and phlebotomy for both adult and pediatric patients is preferred.
- Effective interpersonal, time management, and organizational skills.
- Strong written and verbal communication skills.
- Organizational ability to maintain consistently accurate records.
- Ability to direct information to the appropriate healthcare provider.
- Proficient in computer skills, including word processing, and efficient use of the internet and email.
- Current CPR and BLS (Red Cross and AHA) certification, valid driver’s license, reliable transportation, and automobile insurance.

Provide treatment recommendations for patients who may need more intensive or more specialized mental health care. • Complete pre-conception medication consultation and psycho-education for individuals trying to conceive and currently treated with psychotropic medications. • Perform asynchronous caseload consultations through a review of clinical information prepared by the patient’s care team, as well as review of available medical records. Case review consultations focus primarily on patients who are new to the caseload or who are not improving as expected under their current treatment plan. Provide recommendations for next steps in treatment. • Complete documentation for case consultations and direct patient visits within 24 hours of visit. • Work with care managers to oversee the status and population health of their assigned patient caseload. The psychiatric consultant will use FamilyWell’s registry to track all assigned patients and prioritize patients for case review and treatment adjustment. • Provide the care team with educational support on diagnosing and managing behavioral health conditions. • Be available to provide occasional ad hoc telephone, email, or telehealth consultations to OB providers for complex patients or challenging patient situations within normal working hours. Maintain professional cell phone and email accounts for contact during usual business hours. Check professional accounts daily and respond to email questions/consultations within two business days, sooner if urgent.

• Psychiatric Diagnostic Evaluations for General Mental Health and Substance Use Disorder across all age spectrums. • Psychopharmacological management of all psychiatric diagnosed treatment conditions • Provide medication-assisted treatment to those with OUD in active withdrawal or recovery. • Supportive psychotherapy, motivational interviewing and proficiency in trauma informed approach, harm reduction modeling and patient-centered treatments. • Demonstrate proficiency and care coordination between psych, substance use tx and medical integration. • Direct support for scheduled and walk-in patients • Completion of validated rating scales documentation and treatment plans to assist in determining level of care. • Care coordination for patients including psychological testing, medical referrals, resources, peer supports, housing and other social determinants otherwise identified. • Direct involvement with multidisciplinary team including PCP's, Pediatricians, OB/GYN, counseling, peer support staff and care coordinators.
