• Provide psychiatric and substance use disorder treatment to patients remotely through telemedicine • Conduct Psychiatric Diagnostic Evaluations for General Mental Health and Substance Use Disorder • Manage psychopharmacological treatment for all psychiatric diagnosed conditions • Provide supportive psychotherapy and motivational interviewing • Coordinate care between psychiatric and substance use treatment and other medical services • Complete forms and documentation for patient care needs • Work with a multidisciplinary team including PCP's, Pediatricians, OB/GYN, counseling, peer supports
• Psychiatric Diagnostic Evaluations for General Mental Health and Substance Use Disorder across all age spectrums. • Psychopharmacological management of all psychiatric diagnosed treatment conditions • Data Waivered (or willingness to learn) to provide medication-assisted treatment to those with OUD in active withdrawal or recovery. • Supportive psychotherapy, motivational interviewing and proficiency in trauma informed approach, harm reduction modeling and patient-centered treatments. • Demonstrate proficiency and care coordination between psych, substance use tx and medical integration. • Direct support for scheduled and walk-in patients • Completion of validated rating scales documentation and treatment plans to assist in determining level of care. • Care coordination for patients including psychological testing, medical referrals, resources, peer supports, housing and other social determinants otherwise identified. • Direct involvement with multidisciplinary team including PCP's, Pediatricians, OB/GYN, counseling, peer support staff and care coordinators.

• Provides patient care modalities of evidence-based practices to best meet the clinical needs of the patients. • Review and signature of patient clinical Psychosocial assessment, diagnosis, treatment plan, and consents as needed for the patient in a timely manner. • Documents all services and patient activities appropriately. • Conducts crisis intervention for the individual and family based on the individual’s safety plan. • Participate in regular case planning with the involved Multidisciplinary Team. • Provides case management services to promote recovery and community reintegration as needed. • Performs other duties as assigned
• Provide direct patient care within the Ascend model, maintaining a clinical schedule that balances administrative duties, as determined by organizational leadership. The team lead position is slated for 80% to 85% direct patient care,15% to 20% administrative responsibilities. • Oversight of assigned Therapy direct reports by monitoring productivity, documentation timeliness, and schedule utilization; serve as the first point of escalation for operational efficiencies. • Support onboarding initiatives and integration of new Therapists. • Provide proactive coaching, conduct bi-annual performance evaluations, and assist with professional development opportunities, ensuring a consistent level of accountability is maintained related to patient impact and quality of care. • Address routine clinical questions when appropriate, while ensuring providers follow supervision and CEED guidance channels. • Participate in training, orientation, and quality assurance initiatives, including chart reviews, to ensure documentation is accurate and meets regulatory compliance. • Contribute to policy and procedure development, compliance initiatives, and timely resolution of patient complaints. • Coordinate with Clinical Operations, CEED, Clinical Supervisors and Therapy leadership to optimize interdisciplinary workflows and training programs.

Examples include; CBT, DBT, Solution-Focused Therapy, Psychoeducation (Symptom Recognition), Trauma Therapy, Addiction Therapy, Acceptance and Commitment Therapy, Bereavement/Grief Counseling, etc. • These specific treatment modalities will be implemented in both the individual and group therapy setting, with a focus of engaging patients in their treatment plan. • Review and signature of patient clinical Psychosocial assessment, diagnosis, treatment plan, and consents as needed for the patient in a timely manner. Maintenance of the clinic record, including documentation of activities performed as part of the service delivery process. • Documents all services and patient activities appropriately. Ensures documentation complies with best clinical practices, contracts, regulatory and other legal • Conducts crisis intervention for the individual and family based on the individual’s safety plan. • Participate in regular case planning with the involved Multidisciplinary Team as new information is gathered to ensure the most clinically appropriate patient disposition. • Participate in individual and group supervision as required. Participate in ongoing education, including in-services, training, and other activities to maintain and improve competency. Demonstrate competency through post-testing, skill observation, and performance as assessed by direct supervisor • Provides case management services to promote recovery and community reintegration as needed. • Performs other duties as assigned
