• Counsels scrum teams on agile principles and values to support self-organization, continuous learning, and continuous improvement with assistance from more senior SM's if necessary. • Interprets the agile metrics and reporting framework in order to track and report key information and monitor team progress. • Utilize Agile tools for managing day to day scrum activities • Develops actionable insights from analytics and reports on team productivity, velocity, and quality. • Develops the stakeholder communication strategy to gather and incorporate feedback into process improvements. • Handles various responsibilities to facilitate the scrum process and support the scrum team. • Implements sharing and documentation systems to facilitate sharing through wikis, documentation platforms, or knowledge management tools. • Coaches team members on agile values, principles, and practices to help stakeholders and product owners understand the benefits of adopting an agile mindset with assistance from more senior SM's if necessary. • Interfaces with teams, stakeholders, and external stakeholders to serve as a buffer to potential obstacles.

• Lead and Manage the delivery activities of multiple scrum teams in a manner that aligns with automation strategy set by business counterparts • Anchor and effectively apply the Agile Manifesto, SAFe core values, Lean-Agile mindset and SAFe principles to lead multiple high-performing, cross-functional agile scrum/Kanban teams as the Scrum Master • Work with business teams to shape business requirements and provide ongoing communication through project execution • Ensure development stories, acceptance criteria, assumptions, and all related documentation are timely and complete, supporting development, testing, and user acceptance activities • Assist product owner to create roadmaps and their dissemination to the team(s) • Communicate and facilitate effectively across diverse audiences within and outside of the Sprint Team (key stakeholders, executives) • Work with cross-functional teams (not limited to - architects, application engineers, UX designers, and outsourced vendors) to create a holistic roadmap and planning for product delivery • Manage Sprints - keeping order, process, delivery, priorities accurate and on time • Keep scrum team focused on concrete priorities while keeping outside influences from disrupting the Scrum team and actively identify and remove waste and distractions • Work with Product Owners and QA team to ensure that testing and feedback is completed in a timely manner and meeting the Sprint timelines • Advocate for autonomous, self-organizing, empowered, and continuously improving teams • Continuously strive to coach team and product owner to deliver higher customer satisfaction, improved velocity and delivery predictability, better system performance and enhanced ability to innovate, scale, and meet compliance requirements • Facilitate regular meetings with all relevant stakeholders and project team to discuss/share status and to track progress & health of the project

Job Duties - Collaborates with clinical staff to coordinate and gather pertinent information from patients to support clinical management of chronic conditions. - Increases individual patient engagement, capability, and accountability through regular patient contact. - Provides education, information, and encouragement to patients about available health interventions including diet, fitness, behavioral and social connectivity programs. - Actively manages a patient panel. - Assists patients with program adherence by monitoring the use of the patient’s digital medicine devices and readings (including blood pressure, blood sugar, etc.), adherence to testing and screening appointments, and other lifestyle factors. - Provides education, information, and encouragement to patients catered to their individual needs. Identifies and provides information to patients about available health interventions including diet, fitness, behavioral and social connectivity programs to increase patient engagement, capability, and accountability. - Coordinates with pharmacists, advance practice providers, nurses, physicians and other care providers; maintains documentation as required by law; and actively participates in special projects as needed. - Assists with interviewing potential candidates and training new hires. - Assists with additional departmental projects including development of presentations and education material. - Represents the department at events. - Adapts behavior to the specific patient population, including but not limited to respect for privacy, method of introduction to the patient, adapting explanation of services or procedures to be performed, requesting permissions and communication style. - Performs other related duties as required.
