Clinical Strategy Pharmacist
SmithRx
Independently manage tasks and projects to meet deadlines. Provide excellent written and verbal communication to internal teams as the clinical subject matter expert. Support the clinical strategy team to enhance drug pathways and clinical programs. Collaborate with internal subject matter experts across the organization for clinical program development and expansions. Provide clinical insights for presentations and responses in the pre-sales process and new client implementation. Join sales and implementation calls when clinical insight is needed. Create, update, and provide clinical input for marketing materials for members, brokers, and marketing campaigns. Evaluate trends in the market and work with data teams to analyze claims and model savings for clinical programs for potential and active clients.
Job Requirements
- Bachelor’s and Advanced Degree in Pharmacy, RPh or PharmD
- Current and unrestricted Pharmacist license (any state)
- 3+ years of experience working as a licensed pharmacist in managed care or a managed care residency required
- Professional verbal and written communication (including developing & delivering formal presentations) and interpersonal skills required
- Self-driven, results-oriented work ethic with a positive outlook
- Ability to take initiative unprompted
- Strong project management skills and ability to coordinate/prioritize multiple projects simultaneously to ensure all deadlines are met
- Ability to collaborate constructively and positively with teammates and partners, even under the stress of a fast-paced business environment
- Travel up to 10%

• Make outbound calls to eligible patients and enroll them with Shields Partner Pharmacy • Educate patients on the services offered by the Shields Specialty Pharmacy Program
• The Consultative Pharmacist 2 consults with patients, pharmacies and providers to provide patient care in the form of optimal medication therapy • Advises safe, accurate and timely usage of medications • Designs, implements, evaluates and makes adjustments to medication care plans • Monitors medication therapy and makes adjustments as needed • Works as the medication expert in focused proactive heath campaigns that target improving health outcomes for patients with specific conditions
• Optimize patient outcomes through telephonic interventions • Focus on the tactical execution of medication management services, including polypharmacy, adherence, and pharmacotherapy optimization • Counsel patients via telephonic outreach on medication adherence, address any medication related problems such as adherence barriers, polypharmacy, or high risk medication use, and make appropriate evidence-based recommendations to patients and Practice Care Teams • Perform focused clinical chart reviews as needed to inform patient interventions • Obtain and document patient commitment to medication adherence and address barriers in real time • Escalate complex clinical cases to prescribers or interdisciplinary teams as appropriate • Meet and sustain performance goals (e.g., STARs, HEDIS) • Collaborate with Physicians, clinical and administrative staff to optimize the Medication Management workflow and patient experience • Proactively identify gaps in medication management through patient interactions and translate patient insights into targeted recommendations that drive improvements in practice operations and clinical workflows • Participate in the design, implementation, and evaluation of pilot programs or process changes (“test and learn”) within the outreach model • Serve as a clinical subject matter expert, leveraging clinical pharmacy expertise to support organizational objectives and adapt to evolving healthcare needs • Develop and maintain standardized resources, educational tools, and clinical protocols to support improved medication outcomes and performance on quality measures (e.g., STARS, HEDIS) for practices, clinicians, and patients
