• Provide Empathetic and Consistent Engagement: Serve as the primary point of contact, building strong, reliable relationships with elderly patients through regular, compassionate outbound calls. • Clinical Data Review and Triage: Utilize your medical/social background to monitor and review Remote Patient Monitoring (RPM) data daily, proactively identifying critical trends, early warning signs, and potential clinical concerns. • Patient Education and Support: Expertly educate and guide geriatric patients on the use of RPM devices, ensuring clear communication and comfortable adoption of new technology. • Coordinate Clinical Response: Accurately document all patient interactions and health updates in the EHR/CCM platform, and efficiently escalate acute clinical concerns to the appropriate licensed medical professional or care team member. • Chronic Care Management (CCM) Oversight: Actively assist patients in understanding, adhering to, and navigating their individualized chronic care plans and community resources. • Focus on the Whole Person: Address patient barriers related to social determinants of health (e.g., transportation, medication adherence) that disproportionately affect our aging and remote patient base.
Job Requirements
- Strong Medical Background PREFERRED: A current or previous clinical certification or license (e.g., Certified Medical Assistant (CMA), EMT/Paramedic, Licensed Practical Nurse (LPN), Certified Nursing Assistant (CNA), or equivalent verified healthcare experience)
- Geriatric Care Expertise: Proven experience (1+ years strongly preferred) in successfully working with and caring for the geriatric population
- Exceptional Emotional Intelligence: Demonstrated capacity for deep empathy, patience, and clear, therapeutic communication, especially with complex, chronic care patients.
- Technological Fluency: Competence and comfort in using remote monitoring tools, electronic health records (EHRs), and mobile platforms
- Independent Work Ethic: Strong attention to detail and ability to thrive and manage a high volume of patient interactions independently in a fast-paced, remote setting
