A day in the life ... - Commencing the day… begin your day by reviewing your case load and preparing your technology, ensuring you have all of the right tools available to service your patients - Navigating with ease… using easy and modern technology, you will navigate through your schedule for the day and partner with our clinical in-home team members (Sprinters) that will visit each patient’s home - Creating meaningful bonds… you will have the opportunity to make a warm and welcoming connection with a diverse range of patients as you prepare to collect relevant information and perform various services - Patient-centric, wellness exam… engage in proactive care by conducting thorough health risk assessments, medication reviews, cognitive screenings and empowering patients’ with educational information regarding their health and well-being - Collaborative Patient Care … work directly with Sprinters to evaluate vital signs, arrange blood draws, and carry out necessary tasks to address the specific needs of the patient - Comprehensive Care Coordination and Management… provide comprehensive care coordination and management, including preventive care interventions, medication management, referrals to specialists, community resources, and documentation of findings Skills and Requirements - Board Certified as a Family Nurse Practitioner in California - Active Family Nurse Practitioner License in California - Consistently exhibits the highest levels of professionalism, integrity, accountability, confidentiality, care and compassion to provide high quality health services - Willingness to work in a revolutionary environment that sometimes necessitates last minute problem solving and out of the box thinking - Technologically savvy and comfortable using tools such as laptops or mobile devices for charting and HIPAA secure messaging apps for care coordination - Strong written and verbal communication skills - Ability to work independently or in a team environment - Pass national background check and valid clinical license search Sprinter Health is an equal opportunity employer. This position also ensures facilitation of health care services within a member centric plan of care, under the direction and supervision of an RN. What You'll Do - Will ensure timely appointments and scheduling. - Assist with implementation and facilitation of services members. - Ensure timely member follow up. Interacts and communicates effectively with the member during telephonic contact. - Encourages member participation and compliance in the program. - Assist with all calls to members. Coordinates the delivery of high quality, cost-effective care based on the members- needs and the managed long term support and services model supported by clinical practice guidelines established by the plan. - Run reports to monitor compliance on scheduled services to ensure timely delivery of services and required follow up. - Utilizes care management process for goal directed member care. - Documents accurately and comprehensively based on the standards of practice and current organization policies. - Provides telephonic assistance for activities specific to the program. What You Bring Education/Experience: - High School Diploma/GED required. - Prefers healthcare industry experience. - Requires a minimum of one (1) year experience in home care, discharge - planning, or case management. Preferably with the elderly frail population. - Prefer a minimum of one (1) year experience in the managed care industry. Knowledge: - Requires knowledge of the standards of practice for coordination of services, and delivery of services. - Requires strong knowledge of managed care principles and concepts including Health Plan Effectiveness Data and Information Sheet (HEDIS). Skills and Abilities: - Requires strong organizational skills. - Requires strong oral and written communication skills. - Requires good PC skills, knowledge of working with a Medical Management - system for electronic medical records and the ability to utilize Microsoft - Office applications (Excel, Access, Word, etc.) - Requires good problem solving and conflict resolution skills. - Requires good presentation skills. - Requires ability to work with limited daily supervision. Assess asthma status to decide if an appointment is needed. Have phone calls and text messages with frequent users of the ER to reduce ER usage. This position is Monday through Friday, any time between1 pm - 7 pm, 4-6 hours a day, flexible based on workload and your own family needs. Will provide laptop, will provide a large share-drive document with pre written instructions for the majority of portal questions, as well as Barton Schmitt protocols. Hourly pay rate $25- $42/hr depending on experience. Requirements Must have at least 2 yrs of Pediatric Nursing experience
