We are hiring a Physician for our Complex Care Program to deliver direct virtual patient care while leading interdisciplinary teams. This program serves high-risk patients through an AI-enabled interdisciplinary model combining virtual and home-based care. Our cutting-edge AI technology enhances your clinical expertise, enabling you to deliver exceptional care to your most complex patients. You will serve as the clinical cornerstone of the care team, providing excellent patient care at critical moments including initial visits welcoming them into the program, and transitions from hospital to home. As both clinician and leader, you'll set the clinical and cultural standard for your team while maintaining accountability for medical outcomes across your patient panel through performance data analysis and metric optimization. Your Responsibilities and Impact will include: - Deliver exceptional care through our AI-based platform to a cohort of complex patients across multiple geographies, leveraging video telehealth visits and an interdisciplinary team. - Clinically manage the patients, with a strong focus on complex geriatric conditions (utilizing the 5M framework) such as polypharmacy, falls and dementia. - Responsible for clinical performance of their teams by monitoring and trending outcomes, and working with other leaders to ensure teams are meeting performance metrics. - Assess opportunities to iteratively improve the clinical model, how AI can improve clinical outcomes, and be a change agent to help advance the clinical model. - Engage patients, patients’ caregivers/family, existing provider network & other Devoted programs to provide cohesive, comprehensive care experiences & optimal clinical outcomes. This will include working closely with patients’ PCPs & specialists, in addition to Devoted’s many specialized clinical & psychosocial teams, such as pharmacy, disease management & education, social work & palliative teams. - Utilize our home grown electronic health information system (Orinoco) for visits while also providing feedback on how to improve the interface. - Steward our patient-centric and open clinical culture to our patients and colleagues. Salary Range: $210,000- $315,000 / year The pay range listed for this position is the range the organization reasonably and in good faith expects to pay for this position at the time of the posting. This position will provide day-to-day leadership for a significant portion of the Engagement Center and will be responsible for leading a team of Patient Engagement team members. The Patient Care Coordinator (Account Manager) is the liaison between the senior communities, patients and the practitioners. This role is to also maximize the provider’s visit at the facility while ensuring accurate patient information is documented.
