JOB DESCRIPTION: SUMMARY OF POSITION Centerstone's outpatient clinic in Shelbyville, TN is seeking a therapist to provide therapy to clients with mental health and substance abuse diagnoses. Providers would be responsible for performing initial assessments, diagnosing mental illness, maintaining treatment plans, and completing all clinical documentation in a timely manner. This position is located in a busy community mental health clinic that works with a diverse clientele through in-person and telehealth appointments. Must be comfortable with working with a variety of clients of all ages, sexes, genders, orientations, intellectual abilities, races and ethnicities. Position includes providing direct therapy services for children/adolescents, adults, and families. In this position, therapists are required to complete all clinical documentation: initial intake assessments, treatment plans, progress notes, and internal referrals. ESSENTIAL DUTIES & RESPONSIBILITIES - Develop therapeutic rapport with clients – Meet with clients as scheduled to aid in development of therapeutic relationship. - Diagnostic Assessment – Formulate a provisional diagnostic assessment based on information presented, identify need for risk assessment (to include substance use assessments). - Concurrent Documentation – Completes documentation within 24 hours. - Provide assessment services, individual (and possibly family) therapy to those identified with substance use issues. - Treatment Planning – Developing, writing, implementing, evaluating and revising overall treatment goals/objectives and plans per ongoing assessment of needs and input from the individual based on their identified strengths. - Therapeutic Interventions – Use of empirically based modalities to engage and manage their mental health or substance abuse diagnosis. - Progress notes completed within current agency guidelines. - Responds to supervisory request for urgent care situations and coverage for vacant caseloads. This includes telephones and face to face intervention. - Keeps administrative supervisor informed of all potential issues that could impact the program or the organization. Role Description The Associate Care Navigator will conduct case management on behalf of the client and Southern Scripts ensuring specialty drug spend trend management savings in accordance with client agreement. This position will solution for lowest cost drug while working in a high touch member centric manner, establishing a member relationship as a single point of contact. The Associate Care Navigator will provide targeted, personalized service based on a holistic view of the member, benefits, health information, and engagement. This may include site of service transitions and all other RxCompass component saving methods. This may include both medical and PBM claims.
