JOB DESCRIPTION: SUMMARY OF POSITION This role will take on more of an administrative role within the military services program, providing support services wherever needed. This position would include community outreach, and applicant would need to live in Middle Tennessee in order to participate attend and support leadership with community outreach and advocacy. ESSENTIAL DUTIES & RESPONSIBILITIES - Acts as point of contact between military services and community partner and funders. - Point of contact between military services and community partner and funders. - Manage the data requests that are required as part of our grant funded programs - Will work as a liaison between military services and wounded warrior project answering all inquiries about client referral status, provider change requests, and contact attempts. - Assign referrals to staff daily and will maintain contract compliance regarding contact timelines. - Assist Clinical Services manager with any other administrative tasks as needed. KNOWLEDGE, SKILLS & ABILITIES - Effectively communicate via written, verbal, in person and virtual methods. - Proficient Working Knowledge of REDcap - Proficient Working knowledge of Daybreak EAP Software - Proficiency with Microsoft Word, Excel, Outlook Required - Interest in mental health treatment and developing programs that benefit the nations veterans. - Knowledge and understanding of the principles, practices, methods and techniques of mental health treatment. - The right candidate will not only possess the necessary skills and background, but also will partner and function across Internal Record Keeping, Compliance, and Health Information Management service domains. QUALIFICATIONS Education Level Masters Degree Required. Years of Experience Minimum of 2 years experience working with Veterans and Military Families in a mental health capacity required. Prior mental health environment work experience is required. Job Description The Nurse Navigator is to provide virtual and telephonic support to patients and healthcare providers.
