This role focuses on assessing client needs, creating treatment plans, and delivering ongoing therapy through telehealth. The LMFT will help clients work through relationship issues, family dynamics, emotional concerns, and life transitions in a structured clinical setting. Responsibilities: - Provide telehealth therapy sessions for individuals, couples, and families - Complete psychosocial assessments and develop treatment plans - Maintain accurate and timely clinical documentation - Monitor client progress and adjust treatment approaches as needed - Support clients with relationship challenges, family conflict, anxiety, stress, and related concerns - Coordinate care and make referrals when appropriate - Follow ethical, legal, and professional standards for virtual care

Job Requirements

  • Active Indiana LMFT license in good standing
  • Master’s degree in Marriage and Family Therapy or a related field
  • Experience providing therapy to individuals, couples, and families
  • Strong clinical assessment and treatment planning skills
  • Comfortable using telehealth platforms and electronic documentation systems
  • Excellent communication and time management skills

The Role: As a Care Pathway (CP) Lead - Cardiology, you will be the overall accountable leader for the success of one or more of our care pathways in the journey from pilot through early access (EA). You are entrepreneurial and have the drive to be able to dive into clinical workflows, uncover challenges, make recommendations for technology based solutions in the Viz app, engage clinical stakeholders and drive change management to promote adoption of the technology.
