Community Care is hiring a Case Manager/Care Manager to join our Walworth team. This rewarding opportunity is perfect for someone who enjoys team collaboration, community case management, and being part of a mission driven organization. This full-time opportunity offers a weekday schedule and a $5,000 SIGN-ON BONUS. It is based in Walworth County with telework currently being the norm. Case Manager/Care Manager Job Responsibilities: - Manages an assigned caseload of program members in collaboration with an RN Care Manager - Completes comprehensive member assessments, member centered care plans, & functional screens - Provides ongoing care coordination and quality assurance monitoring of services - Maintains required documentation in adherence to established standards. Case Manager/Care Manager Job Requirements: Education: Bachelor’s Degree in a Human Services Area required. Experience: One year of experience with older adults or persons with physical and or intellectual disabilities in a social service capacity required. Certification: Eligibility for long-term care functional screener certification is required Travel: Regular local travel, requiring use of personal vehicle, valid driver's license, and proof of required insurance coverage.
