Key Responsibilities: - Advanced Clinical Practice (Virtual): Provide comprehensive, client-centered virtual care, including assessing, diagnosing, treating, and following up with patients for a wide range of acute and chronic conditions. This involves conducting virtual video/telephone visits, ordering/interpreting diagnostic tests, prescribing medications within scope of practice, and managing cases across various disease states. - Virtual Care Management: Perform remote patient monitoring and effectively manage patient care through various digital health platforms. - Clinical Leadership & Mentorship: Act as a primary clinical resource, offering guidance, mentorship, and fostering competency development for nursing staff and other health professionals. - Program Development & Quality Improvement: Lead the development, implementation, and evaluation of virtual care policies, protocols, programs, and quality improvement initiatives to enhance service delivery and patient outcomes. - Interdisciplinary Collaboration: Collaborate effectively with nurses, pharmacists, physicians, and allied health professionals to ensure the delivery of integrated, comprehensive, and patient-centered care. - Data Analysis & Reporting: Identify, analyze, and report on clinical data to drive continuous improvement in patient care and virtual health services. - Documentation & Compliance: Ensure all electronic medical records (EMR) are accurate, timely, and fully compliant with privacy regulations, confidentiality standards, and provincial/state legislation governing telehealth. - Operational Support: Assist in the orientation, training, and ongoing support of new team members, and ensure the coordination and availability of necessary equipment for seamless virtual visits. Required Qualifications & Skills - Education: Master’s degree in Nursing or Nurse Practitioner program. - License: Current, valid registration as a Registered Nurse in the Extended Class (NP). - Experience: Minimum 3–5 years of clinical nursing experience, with preference for prior experience in primary care, community health, or virtual care/telehealth. - Technical Proficiency: Competency in using information technology, including EHRs, video conferencing, and remote monitoring equipment. - Leadership: Strong leadership, communication, and decision-making skills.
