Overview Fully remote position after successfully completing 90 day orientation POSITION SUMMARY: The RN Complex Care Manager engages designated patient populations to provide complex care management and chronic disease management and achieve optimal outcomes by assessing, planning, implementing, coordinating, and monitoring interventions. Develops a comprehensive plan of care with the patients, patient designee, providers, and the entire care team to set and monitor goals. Responsible for evaluation and coordinating options and services across the continuum of care. Health care services are coordinated in collaboration with the patient to address the complex medical and clinical needs associated with illness, injury, or chronic disease. Demonstrates the knowledge and skills necessary to provide services to the assigned patient population. Works with the Care Management Division leadership team to ensure alignment with the strategic goals to meet OSF Key Results. Qualifications REQUIRED QUALIFICATIONS: Education: Bachelor's Degree in Nursing must be obtained within 3 years of date of hire into role. Experience: 3 years of caring for patients Care management experience taking care of adult and/or pediatric specializing in chronic, complex illnesses. Broad knowledge of medical and community resources. Positive, empathetic approach and ability to communicate effectively to patients/ families with a variety of health issues, age groups and socioeconomic backgrounds.
