In 2020, Stella Maris opened The Sister Mary Karen McNally, RSM Rehabilitation Center featuring 60 private rooms, state-of-the-art equipment, therapy garden, bistro and Maryland’s only sub-acute rehabilitation center featuring the Bioness Vector Gait & Safety System. The Admissions team, led by the Director of Business Development, Sales & Marketing, consists of Referral Coordinators and Admissions Nurses, working together to review referrals from local hospitals, and other entities, seeking sub-acute rehab and long-term care placement. Responsibilities The essential function of this position is to complete pre-admission clinical assessments for referrals to Short-Term Rehabilitation and Long-Term Care. SHIFT: Weekend (9am-5pm) Remote - Establishes a positive working relationship with referral sources including referring hospital discharge planners, case managers and social workers throughout the assessment process. - Obtains all information necessary to make admissions decisions, including patient’s clinical data and insurance information (as needed), in collaboration with Admissions Referral Coordinators. - Is comfortable and well-versed using technology such as dual desk-top monitors, and web-based referral systems to communicate with referring hospitals (MD Portfolio, NaviHealth, CarePort (formerly AllScripts) and EPIC - Determines which program or service best meets the patient’s needs and desires efficiently and accurately based on established criteria. Considers prohibitive factors such as cost of care, medications and other special needs when assessing. - Communicates to appropriate team members necessary information, to include nature of skilled services, equipment needs and more in an effort to remove barriers to admission and ensure a smooth admissions process for the patient. - Responds appropriately and knowledgeably to all questions about Stella Maris services and programs with a thorough knowledge of Stella Maris' clinical capabilites. - Supports proper referral/lead management through utilization of Point Click Care CRM (Customer Relationship Management Database) in collaboration with Referral Coordinators. Requirements • LPN or RN licensure in Maryland Required • Minimum of 2 years nursing experience • Valid driver’s license and reliable transportation

Responsibilities - The ideal candidate will have at least 5 years of MDS experience including ICD 10 coding for PDPM in a LTC/SAR setting. - Emphasis on choosing the highest ranking clinical category possible and capturing NTAs for maximum reimbursement. - Hours will vary according to the number of admissions each day. - Responsible for identifying payer source, opening all Entry MDS and setting ARDs for Admission and 5-day assessments. - Entering and ranking all ICD 10 codes based on the hospital discharge summary and accompanying documents. Qualifications - RN License - Minimum 5 years of experience with MDS required
