SUMMARY The Hospice Project Manager plays a vital role within the Implementation Team, coordinating efforts and collaborating with senior leadership to ensure the successful execution of new joint venture partnerships. This position is responsible for developing and managing project plans aligned with leadership directives, overseeing key tasks from due diligence through full implementation. The Project Manager engages stakeholders, ensures accountability for project milestones, and maintains structured documentation to support an efficient implementation process. Beyond implementation efforts, this role will also contribute to additional project management initiatives as needed. Travel Requirement: Up to 60% during implementation phases. KEY RESPONSIBILITIES - Serves as a liaison between internal teams and JV partners, providing updates on implementation progress and due diligence. - Collaborates closely with ASHN leadership and implementation teams to drive strategic goals and ensure successful project execution, with a strong focus on hospice. - Partners with senior leadership and stakeholders to define project scope, goals, and deliverables aligned with business objectives. - Oversees all aspects of hospice project planning and implementation, ensuring efficiency and alignment with organizational priorities. - Facilitates engagement on project outcomes through structured meetings and regular progress updates. - Holds team members accountable for completing assigned project tasks within established timelines. - Identifies project risks and implements proactive mitigation strategies to ensure smooth execution. - Supports JV implementations and Go-Live stages, including potential on-site involvement and travel as required. - Coordinates and schedules implementation-related meetings, including agenda preparation and documented minutes. - Maintains high standards of communication with JV partners and internal staff to foster transparency and collaboration. - Organizes and oversees all project documentation, including contracts, schedules, budgets, and communication records. - Assists leadership in applying structured decision-support methodologies to solve complex problems effectively. - Partners with training teams to facilitate outreach and education on project management principles. - Demonstrates company core values through professional conduct and cultural alignment. - Applies continuous improvement methodologies to contribute to the refinement of project management methodologies, tools, and best practices. - Performs additional responsibilities as assigned. QUALIFICATIONS - Minimum 2+ years of Project Management experience is required, multi-site experience is preferred - Proven experience managing complex projects, preferably in joint ventures and/or healthcare partnerships - Strong background in due diligence, stakeholder engagement, and implementation strategies - Exceptional relationship building, communication and presentation skills - Ability to organize, systematize complex work, and prioritize effectively in a fast-paced environment - Strong analytical and problem-solving skills - Expert level data tracking, analyzation and reporting skills - Knowledge of Medicare, Medicaid and State/Federal guidelines for compliance of patient clinical care and for eligibility and coverage - Proficient in Project management using software such as: Microsoft Office (Excel, Outlook, Power Point, Visio, and Word) and SmartSheet - Excellent leadership and collaboration abilities EDUCATION AND CREDENTIALS - Bachelor’s degree in Business Administration, Project Management, or a related field (Master’s preferred) - PMP (Project Management Professional) certification - Desired certification in Six Sigma We’ll help you put your passion for patient care to work. The Healthcare Operations Manager leads the Care Coordination team and ensures operational workflows—from member intake and eligibility verification to order support and service resolution—are executed accurately and consistently. Because operational accuracy directly impacts healthcare claims and reimbursement, this role also works closely with Finance and Revenue Operations to minimize operational errors that could lead to claim denials or payment delays. This is a hands-on leadership role focused on operational rigor, accountability, and continuous improvement within a regulated healthcare environment. Responsibilities: 1. Care Coordination Team Leadership & Performance Lead and manage the Care Coordination team responsible for member intake, onboarding, and ongoing program support. Responsibilities include: - Hiring, coaching, and managing Care Coordinators and Member Support Coordinators - Defining and enforcing operational workflows and service-level expectations for: - Intake calls and eligibility verification - Member profile and address validation - Meal order support and modifications - Escalation handling and issue resolution - Monitoring team performance metrics including: - Intake accuracy - QA scores - SLA adherence - Member satisfaction - Conducting regular 1:1s, performance reviews, and coaching sessions - Acting as the escalation point for complex or high-impact member issues 2.
