Role Description The Coder (Clinic - II) performs coding review for primary care or specialty services for ThedaCare Physician Services to accurately reflect services rendered. Reviews and processes charges using industry standard methodologies (CPT, ICD-10-CM, HCPCS), abides by Standards of Ethical Coding (AAPC/AHIMA), and complies with official coding guidelines and other regulatory requirements. Audits medical record documentation for completeness and accuracy. Educates and/or collaborates with providers and other clinical staff when completion or clarity is needed. SCHEDULE: - Full time, benefit eligible - 40 hrs/week - Remote eligible and flexibility in schedule - Strong E/M experience is required and surgical experience is preferred. KEY ACCOUNTABILITIES: - Reviews and/or assigns CPT and/or diagnosis codes (ICD-10-CM, including HCC risk adjustment) for primary care and/or specialty clinic services (E&M, office procedures, inpatient hospital rounding, OB services, outpatient general practice procedures, etc.) with a high degree of accuracy. - Performs documentation reviews with the use of department auditing tools, shares results with providers and auditing team, and identifies educational opportunities. - Collaborates with providers, clinical team members, and/or other departments when completion or clarity is needed within the medical record to accurately complete coding functions. - Manages and maintains coding inventory responsibilities related to payer denials and claim edits to ensure timely reimbursement for services provided. Role Description The RN Clinical Nurse (Ambulatory) provides patient-centered, specialized, evidence-based nursing care across the continuum through an interprofessional approach to treatment, research, education, and advocacy. Summary : The Coder (Hospital - II) reviews patient medical record documentation and assigns and sequences ICD-10-CM diagnosis codes and ICD-10-PCS or CPT/HCPCS procedure codes, when applicable, to inpatient, observation, and/or outpatient surgical accounts, abiding by the Standards of Ethical Coding as set forth by the American Health Information Management Association (AHIMA) and in compliance with official coding guidelines and other regulatory requirements. Communicates with providers when needed to obtain complete and specific documentation to accurately assign codes. Follows up and obtains clarification on charge issues. Works with providers and/or clinical documentation improvement (CDI) specialists to achieve a high level of documentation specificity. Maintains department standards for timely coding completion. QUALIFICATIONS: - High School diploma or GED preferred - Must be 18 years of age - One year of hospital coding experience. - Current or obtained within one year of hire — Certified Coding Credential obtained through AHIMA or AAPC, or RHIT (Registered Health Information Technologist/AHIMA), or RHIA (Registered Health Information Administrator/AHIMA). Minimum certification requirement is either CCA (Certified Coding Associate/AHIMA) or CPC-A (Certified Professional Coder - Apprentice/AAPC). Inputs verification data to complete in-house claims generation of billing forms. - Re-bills accounts when new information is received requiring account updates with appropriate demographic and third party information to ensure payment.
