POSITION: The state of Indiana Clinical Provider (NP/IBCLC or PA/IBCLC) is responsible for ensuring direct patient care is evidence-based and aligns with SimpliFed's maternal health program requirements. Provider will conduct lactation-related clinical assessments, oversee remote patient monitoring, administer CPOE orders within scope of practice, and screen for maternal mental health. ROLE: The role of the Clinical Provider (NP/IBCLC) is expected to uphold the highest standards of care for the company and its patients. Providers will be required to engage in telehealth, virtual video, face-to-face appointments with patients to provide care and assist patients with their personal goals and those goals prescribed by their health care team(s). Providers are responsible for all clinical decision-making in alignment with SimpliFed's clinical protocols associated with the provision of Professional Services. VENUE: This role works full time remotely using SimpliFed approved devices. POSITION SUMMARY: The role of the SimpliFed Clinical Provider (NP/IBCLC) will conduct professional services consultations with patients via telehealth. Providers, based on their respective professional fields, will assess patients and provide education on topics such as lactation, infant feeding, hypertension, gestational diabetes, anemia, thyroid levels, and maternal mental health to name a few. Providers will document their findings in the exam form, provide proper resources to patients, and schedule patient follow up appointments, based upon need. Providers will make all necessary referrals to outside and referring providers based upon the need of the patient. REQUIREMENTS: - Master’s or Doctoral degree in respective clinical field (Nurse Practitioner) - Professional licensure in respective clinical field (Nurse Practitioner), in good standing with the governing board. - Professional certification as an International Board Certified Lactation Consultant (IBCLC) or Certified Lactation Counselor (CLC), in good standing with the governing board. - 3 years of direct clinical experience in hospital, clinic, or other healthcare setting with a minimum of 2 years experience as a CLC/IBCLC. - Active MMIS (State Medicaid) Credentials Preferred. - Interest in maternal health and improving outcomes. - Provide education and clinical support services to empower families to make informed decisions around baby feeding and infant care. - Ability to work independently and make sound decisions aligned with SimpliFed’s policies. - Strong active listening skills and sound judgment. - Demonstrated written and oral communication skills, as well as strong charting skills. - Knowledge of clinical workflows, healthcare quality and compliance standards, EHR data management, clinical documentation, HIPAA, and related regulations. - Ability to accept constructive feedback and make improvements to better serve SimpliFed patients, partners, and mission. POSITION SPECIFICS: - SimpliFed providers will see patients for a variety of matters in the maternal health and perinatal space. Services provided may include, but not limited to: - Lactation and Infant Feeding Services provided by IBCLC and CLC credentialed professionals. - Remote Patient Monitoring for Blood Pressure and Gestational Diabetes Nurse Practitioner, Physician Assistant, and Registered Nurse credentialed professionals. - Perinatal Mental Health provided by all SimpliFed providers and Credentialed Perinatal Mental Health Counselors (PMHC) - Other issues in the maternal vacuum of care such as, but not limited to, nutrition, anemia, thyroid issues. - SimpliFed Providers will communicate with patients via the company's electronic medical record system and virtual video platform. - SimpliFed Provider communications should focus on clinical relevance and empower patient health literacy and evidence-based decisions. - Review of patient’s intake forms, complete a history and virtual physical evaluation of the pregnant or postpartum patient. - Conduct Perinatal Mental Health Assessment using standardized assessment instruments and tools provided in the Exam Forms as well as general observation of a patient's current mental state. - Provide education on topic(s) associated with the appointment type. - Obtain relevant patient medical history using the intake form or other forms as defined by SimpliFed. - Complete Exam Forms in full, including, but not limited to, a record of the start and end times of the visit as well as relevant medical history, intake, baby wellness, diagnosis and procedure codes for the visit, by checking the appropriate boxes on the exam form - Complete Documentation of all events, observations, evaluations, assessments, and instructions relevant to the visit, the same day as the completed appointment. - Develop an individual Care Plan for each patient which includes written instructions, resources, and a plan for follow up. - Complete Referrals/Coordination of Care (if necessary) and send to the proper entity. - Scheduling subsequent appointments with patients prior to completing the current appointment. WORK ENVIRONMENT: Work is performed virtually. Comfortable working with remote productivity communication tools is required.

• Conduct customer discovery, user experience analyses/observation (quan + qual), and lightweight usability studies to understand ways to improve our patient and customer experience • Translate these findings into requirements to implement throughout our patient experience • Strong knowledge of APIs, AWS, Databases, Kubernetes/Docker, and JavaScript • Define and maintain the product vision, strategy, and long-term roadmap. • Analyze market trends, customer feedback, and competitive landscape. • Translate business goals into clear, prioritized product initiatives. • Write detailed PRDs (Product Requirements Documents), user stories, and acceptance criteria. • Create detailed project plans, sprint schedules, and release calendars • Maintain Jira backlog hygiene and sprint predictability. • Coordinate and implement these project plans in collaboration with external partners and internal teams • Partner closely with engineering to refine the scope and ensure technical feasibility. • Own the end-to-end product lifecycle: planning → development → testing → launch. • Communicate status, risks, and product changes clearly and in a structured way.

• Collaborate with the CEO and executive team to define and implement company strategy, annual/daily/monthly KPIs, and long-term goals. • Translate strategy into actionable plans across business, clinical, and operational functions. • Lead and continuously evolve SimpliFed’s operational strategy, infrastructure, and systems for scale. • Drive cross-functional alignment around KPIs and ensure performance is monitored and all members are held accountable and responsible for KPIs. • Partner with finance to develop and manage operational budgets, ensuring efficient resource allocation and cost controls. • Collaborate with the CTO to ensure operational processes are supported by scalable technology and data systems, driving automation and actionable insights. • Directly oversee all departments excluding, Commercial (CCO) and Technology (CTO). • Support a distributed, virtual team environment by maintaining operational clarity, ownership, and accountability. • Lead company-wide operating rhythm including weekly team “backstage” meetings, all-hands, and planning cycles. • Oversee hiring operations, onboarding, performance reviews, and workforce planning in collaboration with team leads. • Ensure execution of a consistent and scalable patient experience and clinical service delivery model. • Maintain and evolve clinical protocols, SOPs, and backstage systems supporting service operations. • Champion operational excellence that enhances both patient outcomes, employee satisfaction, and provider satisfaction as we grow and scale. • Maintain organizational compliance with all regulatory, contractual, and legal requirements—including Medicaid and commercial payer requirements. • Ensure all contracts, employee agreements, policies, and operational documents are up to date and securely managed. • Manage operational risk, audit readiness, and security best practices with a high standard for data integrity and family privacy. • Develop and maintain business continuity plans to ensure uninterrupted service delivery during crises or system outages. • Own and optimize the tools, workflows, and documentation systems that power SimpliFed’s day-to-day operations (e.g., Google Workspace, Slack, Jira project management tools). • Partner with functional leaders to scope and manage major cross-functional initiatives and special projects. • Drive continuous improvement by applying human-centered design, systems thinking, and agile principles.

• Own the day-to-day execution of email, SMS, and website marketing efforts • Set up and maintain lifecycle campaigns across email and SMS • Monitor performance and ensure website functions smoothly from a marketing perspective • Troubleshoot issues like broken links, 404s, and form flows • Collaborate with marketing and clinical/content teams to implement copy and messaging • Identify trends, gaps, and opportunities to improve engagement and conversion • Run A/B tests and use data to recommend and implement iterative improvements • Manage marketing-related website updates in Webflow

• Manage employee devices, accounts, and access (laptops, mobile devices, SaaS tools) • Administer identity and access management (SSO, MFA, role-based access) • Support onboarding and offboarding while maintaining security and compliance • Maintain and troubleshoot networks, endpoints, and cloud-based systems • Monitor system health, patching, and updates • Respond to IT support requests and resolve issues quickly • Assist with HIPAA, SOC 2, and security audits • Document IT processes, policies, and procedures

• Support and maintain cloud infrastructure on AWS • Use Terraform to provision and manage infrastructure as code • Assist with CI/CD pipelines for application deployments • Monitor system performance, availability, alerts, logs, and events • Help implement security best practices (IAM, least privilege, logging) • Troubleshoot infrastructure and deployment issues • Work closely with engineering and IT teams to support production systems • Document security controls, infrastructure, runbooks, and operational procedures

• Build and maintain backend and GraphQL APIs using Node.js • Develop and maintain web and mobile features with React.js and React Native • Work directly with product and design to ship features quickly • Debug issues across the stack and improve performance • Write clean, maintainable code and contribute to code reviews • Help deploy and monitor applications in production • Work with databases (SQL or NoSQL) to read and write data

• Leads practice day to day strategy and execution by implementing operational frameworks, policies and performance systems that drive quality and scalability. • Own program delivery end-to-end, including existing and new service implementations, ensuring projects are completed on time and aligned with strategic goals. • Translate performance data into action by developing improvement plans, optimizing workflows, and continuously refining operational processes. • Drive cross-functional collaboration with clinical, product, finance, commercial and senior leadership teams to align services with patient needs and client commitments. • Build and optimize operational infrastructure by identifying automation opportunities, improving tooling, and developing real-time reporting to support data-driven decision-making. • Manage internal and external key stakeholder relationships and ensure high service quality, successful implementation and fulfillment of service level agreements while identifying growth opportunities. • Lead and develop high-performing teams through hiring, coaching, and succession planning, fostering accountability and operational excellence. • Anticipate and respond to organizational and market factors that impact our practice proactively adjusting strategy and execution. • Proactively identifies strategies to ensure SimpliFed hits any provider-driven service level agreement obligations set forth by commercial agreements. • Perform other duties as assigned.
