Role Description The Quality Assurance (QA) Supervisor is a Registered Nurse responsible for overseeing the accuracy, completeness, and regulatory compliance of all OASIS, HOPE, and Plan of Care (POC) documentation for home health and hospice services. This role ensures patient records accurately reflect diagnoses, care plans, and skilled interventions in accordance with Medicare, state, and accreditation standards. The Quality Assurance Supervisor supports clinical excellence and optimal reimbursement through staff education, auditing, and process improvement initiatives. In addition, this position is responsible for managing Quality Assurance nurses, overseeing QA workflows, and ensuring compliance with corporate standards in support of the organization’s overall mission and goals. In this role you will be responsible for: - Participates in the organization’s strategic planning activities. Focuses attention on quality objectives. - Aids with the development, implementation, and evaluation of policies, procedures, SOPs, forms, quality improvement monitors, and other components of program infrastructure every year and as needed. - Directs all aspects of home care and hospice QA operations in accordance with the mission and values of Optimal Care, legal and regulatory requirements, payer and accreditation standards, customer expectations and organization policy. - Supervises and coordinates resources to manage timely completion of QA workflow including submission of OASIS and HOPE. - Utilizes principles of personnel management, legal considerations and fair employee practices. - Coaches and directs staff based on findings from formal and informal needs assessments. - Conducts performance appraisals, accurately and as per policy. Ensures all documentation is accurate, complete, and timely and adheres to Medicare guidelines. - Maintains adequate staffing levels to manage QA workflow. - Ensures compliance with the requirements of state licensure, Medicare certification, and any other applicable oversight agencies through policy and procedure development, staff education, and ongoing monitoring activities. - Provides input to related administrative/computerized systems and monitors and processing of data relating quality care and measurable outcomes. The Triage Registered Nurse is responsible for the coordination of care during the episode of health care treatments. In this role you will be responsible for: - Communicates significant findings, problems, and changes in condition or environment to the physician and other personnel involved with patient care - Initiates a plan for patient safety, using the patient, family, and community resources. - Verifies the plan of care for the episode according to physician's orders, assessment data, and established standards and guidelines. - Assumes primary responsibility for "after hours supervisor" including assessing, planning, coordinating, implementing, and evaluating the plan of care for patients and families. - Manages and directs field team and weekend office staff, to ensure the delivery of safe, effective, and appropriate hospice services - Coordinates services to ensure the availability and competency of staff to respond to organization referrals and provide assessment visits within 24 hours of referral  Required Qualifications - Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing - Current State License as a Registered Nurse - 2-3 years experience as a registered nurse in an appropriate clinical care setting - 1 year of supervisory experience - Nursing skills as defined by accepted nursing standards - Requires interpersonal skills and ability to manage patient care episodes - Demonstrates ability to assess and respond to the needs of patients, families, and caregivers in varied settings and to function effectively in the home or nursing facility environment - Demonstrates skill in nursing practice, ability to cope with ill, disabled, terminal patients, family emotional stress, and tolerance of individual lifestyles - Demonstrates ability to handle multiple priorities, documentation requirements, and deadlines. - Current and valid driver’s license - Reliable transportation and valid automobile insurance coverage - Sit, stand, bend, lift, and move intermittently - Ability to lift at least 35lbs and bear the weight of an average adult effectively Desired Qualifications - BSN preferred - Experience as a nurse in a hospice or home health environment preferred - Familiarity with Home Care Home Base (HCHB) Location - This position will work remotely - Candidates must live in Michigan, Ohio, and Indiana Hours - This position is per diem - Flexible to support days or overnights - Biggest need with be Friday/Saturday/Sunday days Pay Range $31—$35 USD Background Screening Optimal Care conducts a background screening upon acceptance of a contingent job offer. The Quality Assurance Nurse is also responsible for the development and management of the organization’s quality initiatives and improvement. Role Description The Physician Services Care Advocate is a professional who provides services to patients and those supporting them who are navigating the complex healthcare continuum. The advocate works directly with patients (or with their legal representatives) to ensure they have a voice in their care and information to promote informed decision making.
