Role Description The Medical Records Coordinator is responsible for managing and maintaining patient medical records in compliance with organizational policies, HIPAA regulations, and applicable state and federal guidelines. This position ensures timely, accurate, and secure processing, storage, and retrieval of medical records and other third-party documentation to support quality patient care, billing accuracy, and regulatory compliance. - Maintain and organize electronic medical records with accuracy and confidentiality. - Process incoming and outgoing medical record requests, and other third-party documentation requests in accordance with HIPAA and company policy. - Ensure timely scanning, indexing, and filing of clinical documents into the Electronic Health Record (EHR) system. - Work collaboratively with clinical and administrative staff under the direction of Compliance to ensure documentation is complete and accurate. - Monitor and audit medical records for compliance and quality assurance. - Assist in responding to audits and chart reviews from internal and external stakeholders. - Serve as a point of contact for medical record inquiries from patients, providers, and third-party requestors. - Coordinate release of information requests, ensuring proper patient authorization and documentation. - Manage retention and destruction of records in accordance with policy and regulatory standards. - Support administrative functions including data entry, reporting, and documentation tracking as needed. Qualifications - High school diploma or equivalent required; Associate degree or certification in Health Information Management preferred. - Minimum of 2 years of experience in a medical records or health information role, preferably in a healthcare setting. - Knowledge of HIPAA regulations and medical terminology. - Experience with EHR systems (e.g., Athenahealth, Epic, or similar). - Strong attention to detail, organizational, and time management skills. - Ability to handle confidential information with discretion and professionalism. - Excellent communication and customer service skills. Job Responsibilities - Complete audits according to an established audit schedule and audit procedures. - Audit patient records for completeness, accuracy, and appropriateness, including: - Ensuring records include the minimal required data. - Comparing source data to the records for accuracy. - Analyzing select data in detail. - Audit patient records and processes related to Geode’s QI/QA program. - Prepare records for audits performed by other staff, including: - Medical director chart reviews. - Patient reviews. - Imminent and eligible classification reviews. - Geode Health internal audits. - Identify opportunities to improve the quality of records, efficiency of processes, and increase performance. - Maintain detailed auditing data, including: - Workload details. - Documented audit findings, recommendations, and corrections. - Inform and educate staff of audit findings, recommendations, and corrections. - Report audit findings, recommendations, and corrections to internal leadership and complete follow-up activities as directed. - Monitor and follow up on requested audit corrections to ensure completion. - Develop audit and documentation/recordkeeping policies, procedures, and workflows. - Organize root cause analyses (RCAs), take minutes, and implement action items from RCAs. - Establish auditing goals, expectations, and benchmarks and ensure these are met or exceeded. - Create and/or adopt existing tools, systems, and methods to organize and catalog a variety of auditing data. - Perform analysis on auditing data to generate metrics and other tracking and trending analytics. - Interpret and monitor metrics and analytics to identify meaning, trends, and compliance towards auditing goals, expectations, and benchmarks. - Prepare and present analytics to internal leadership and external audiences ranging from front-end staff to executive leaders. Qualifications - Bachelor’s degree in business administration, science, health care, public health, or other life sciences related field. - Registered Nurse (RN) license required. - Three years of related quality assurance experience; within Behavioral / Mental Health preferred. - Ability to work with highly detailed, complex, and emotionally sensitive information. - Ability to maintain a high degree of discretion, confidentiality, protection, and integrity. - Ability to learn complex processes, medical terminology, technology systems, and progressively develop data analytics skill set. - Ability to prioritize and complete multiple complex work assignments. - Ability to work independently. - Knowledge of quality assurance, quality systems, internal auditing, and/or quality improvement preferred. Role Description The Clinical Licensing Specialist (Program Specialist) supports regulatory and credentialing operations by coordinating provider licensure tracking, verification, and renewal processes. This role ensures providers maintain appropriate licensure throughout the hiring process and active employment and supports regulatory documentation requirements related to collaboration agreements and provider licensure compliance. This role is responsible for managing and coordinating the collaboration agreement process across all states, ensuring alignment with varying state-specific regulatory requirements and maintaining accurate, compliant documentation.
