• Serve as the first point of contact for prospective patients. • Contact referred patients and consumers who have requested more information. • Explain Custom Health services in a clear, supportive, and professional way. • Build trust and rapport with patients and caregivers. • Answer questions and provide education regarding available programs. • Deliver a positive and supportive patient experience. • Assess patient eligibility, interest, and program suitability. • Verify coverage and help patients understand available services and next steps. • Support enrollment and onboarding readiness. • Identify and address barriers that may prevent enrollment. • Manage patient progression through qualification. • Maintain accurate records within CRM systems and patient tracking tools. • Organize and track patient progress through the onboarding process.

Job Requirements

  • 3+ years of experience in healthcare, patient coordination, customer care, social work, or a related field.
  • Exceptional communication and relationship-building skills.
  • Professional telephone presence and comfort managing patient-facing conversations.
  • Strong organizational and time management abilities.
  • Comfortable managing a high volume of patient interactions and follow-ups.
  • Experience working with CRM systems preferred.
  • Strong attention to detail and follow-through.
  • Proficient with computers, software programs, and remote work tools.
  • Comfortable working independently in a remote environment.

Role Description As a Patient Support Coordinator, you’ll be the warm, knowledgeable voice that patients rely on from their very first interaction with Montu UK. Acting as the front line of our patient experience, you’ll handle around 450 calls each week, spending roughly five minutes per call guiding patients through a variety of needs - from answering questions to providing reassurance and practical support. Salary: £27,000 40 hour working week. (5 days - Remote - in the UK) Hours: 7:30am - 4:30pm/9:30am-6:30pm Some weekend working is required across a 5 day week.
