Job Description

Network Development Coordinator

Trillium Health Resources

• Oversee and maintain project plans, project timelines, milestones, stakeholder communications management, and other day to day project activities. • Has autonomous decision-making authority on projects that impact the agency. • Lead assigned project(s) with team(s) both internal and external through initial development until project completion and released from Network Development for ongoing monitoring in other departments across the organization. • Provide project status updates/reports using dashboards and/or Smartsheet at regular intervals during all projects. • Ensure all assigned project components are completed in a timely manner. • Address network needs via specific provider recruitment and the publication of Open Enrollments, Requests for Proposals (RFP), Request for Application (RFA) or Request for Information (RFI). • Independently complete Requests for Proposals, Application and/or Information using knowledge by Subject Matter Expert’s (SME) to obtain the service specific to the needs of the population, area and organization.

Job Requirements

  • High School Diploma/GED and six (6) years of progressive experience in the behavioral health (mental health/substance use), Intellectual-Developmental Disability/Traumatic Brain Injury, and/or Physical Health Care field
  • OR Associate’s degree in a Human Services field and four (4) years of progressive post-degree experience in the behavioral health (mental health/substance use), Intellectual-Developmental Disability/Traumatic Brain Injury, and/or Physical Health Care field
  • OR Bachelor’s degree in a Human Services field and two (2) years of progressive post-degree experience in the behavioral health (mental health/substance use), Intellectual-Developmental Disability/Traumatic Brain Injury, and/or Physical Health Care field
  • OR Equivalent combination of education/experience.
  • Must have a valid driver’s license.
  • Must reside within North Carolina.
  • Must be able to travel within catchment as required.

Role Description We are seeking a Health Engagement Specialist to join our team! In this role, you will be responsible for outreaching to patients in need of important preventive care services, including those who may be overutilizing the emergency room or those transitioning from hospitals to home or other care settings. What you’ll do - Conduct a high volume of outbound calls to patients, including many who have not previously encountered Diverge Health. - Field inbound calls from patients responding to outreach from either Diverge Health or referrals from their primary care provider, or from primary care providers attempting to coordinate patient services with Diverge Health. - Identify and support patients who are overutilizing the emergency room by guiding them toward more appropriate care options. - Reach out to patients undergoing care transitions (e.g., from hospital to home following a hospital discharge) to ensure they have follow-up care in place, educate patients about the importance of preventive care and the need for regular visits with their provider. - Assist patients in scheduling appointments, navigating care options, and resolving barriers to access. - Build trust with patients and tailor conversations to meet their individual needs. - Maintain accurate and organized records of patient interactions using CRM systems (e.g., Salesforce). - Meet or exceed daily outreach and engagement performance metrics.
