In this role, you will provide expert veterinary care through virtual consultations, offering advice, diagnosis, and treatment plans for pets via video, phone, and written communication. Conduct remote consultations with pet owners, offering professional advice, diagnosis, and treatment plans based on medical history and symptoms. Provide guidance on pet health topics including nutrition, preventive care, and minor medical issues. Collaborate with veterinary technicians to review patient history and assist in preparing recommendations for telemedicine consultations. Issue prescriptions for medications when necessary, ensuring compliance with telemedicine and state regulations. Document consultation details accurately in the system, maintaining clear, thorough medical records. Respond to follow-up inquiries from pet owners, offering clarification or adjustments to treatment plans as needed. Ensure that all telehealth services are delivered in accordance with PAW Health Network’s policies, procedures, and applicable veterinary regulations.

• Reaches out to members telephonically to assist with referrals, authorizations, HHC, DME needs, medication refills, make provider appointments and follow ups, etc. • Creates cases, tasks, and completes assessments in Case Management module for all Hospital and SNF discharges. • Works as a team with the Case Manager to engage and manage a panel of SNP members. • Establishes relationships with members, earns their trust and acts as patient advocate.

• Conduct outreach to newly referred clients and log activity in Salesforce • Conduct 8-12 intake visits daily, ensuring efficient and accurate assessments • Assist clients in completing initial intake visit including a psychosocial assessment and care plan with goals of care • Review medical charts and document client medical needs and suggested interventions • Communicate critical information and client preferences to the care team for personalized care planning • Utilize evidence-based practices, such as Motivational Interviewing, Harm Reduction, and Trauma-Informed Care principles • Attend weekly Team Operations Meetings • Maintain a structured and organized schedule to optimize client intake and engagement • When unscheduled, perform proactive outreach to no-shows or clients who have not been contacted previously • Stay available for warm transfers from field teams, ensuring same-day responses • Thrive in a fast-paced, dynamic environment with an emphasis on meeting key performance metrics
