Care Coordinator, Bilingual Spanish
Alignment Health
• Reaches out to members telephonically to assist with referrals, authorizations, HHC, DME needs, medication refills, make provider appointments and follow ups, etc. • Creates cases, tasks, and completes assessments in Case Management module for all Hospital and SNF discharges. • Works as a team with the Case Manager to engage and manage a panel of SNP members. • Establishes relationships with members, earns their trust and acts as patient advocate.
Job Requirements
- Minimum 1 year experience working in Health Care IPAs.
- Minimum 1 year experience entering referrals and prior authorizations.
- Required: High School Diploma or GED. Bachelor's degree or four years additional experience in lieu of education.
- Bilingual: Spanish or Vietnamese preferred.
- Ability to read and interpret documents such as safety rules, operating and maintenance instructions and procedure manuals.
- Basic Computer Skills, 25 WPM (Microsoft Outlook, excel, word)

• Serve as the primary emergency contact for all HHA and PDN cases outside of standard office hours • Respond promptly to incoming calls related to staffing, attendance, patient needs, and caregiver issues • Monitor and verify caregiver attendance for all scheduled HHA and PDN cases • Document all calls, incidents, and case-related updates in HHA Exchange in a timely and accurate manner • Provide coverage for all missed visits by securing appropriate caregiver or nursing replacements • Staff all open cases and ensure continuity of patient care • Communicate with caregivers regarding scheduling changes, call-outs, and attendance concerns • Escalate urgent clinical or operational matters to appropriate management as needed • Maintain detailed and accurate call logs for all after-hours activity • Ensure all documentation and service updates are entered in compliance with agency protocols
