Role Description Under the supervision of a licensed pharmacist, this role supports high-quality, compassionate patient care while adapting to the dynamic healthcare environment. Responsibilities include: - Obtaining and reviewing post-discharge medical records - Navigating multiple electronic medical record systems (EMRs) - Analyzing clinical documents such as labs, history & physicals, provider notes, and discharge summaries - Conducting outreaches to members to update medication lists - Coordinating with providers to confirm care relationships - Following up on clinical recommendations - Identifying medication discrepancies - Reviewing prescription claims data - Preparing detailed reports for pharmacist reviews - Collaborating across care teams to support coordination efforts - Working independently in a fast-paced, evolving setting If you reside near Dapper, UT, you will enjoy the flexibility to telecommute as you take on some tough challenges.
