• Provide In-Home Health Evaluations • Engage with patients in their homes • Build trusted relationships to improve health outcomes • Conduct virtual visits as needed • Provide all Signify Health services as indicated including Diagnostic & Preventive Services products

• Provide In-Home Health Evaluations engaging with people in their own homes • Conduct medication and medical history reviews and physical evaluations • Use technology for evaluations without prescribing medicine or altering treatment regimens • Travel locally within licensed states as needed

• Present a positive and professional demeanor to clients, health plan members, clinicians, and co-workers • Leverage multiple internal and external systems to facilitate member scheduling of appointments • Assist health plan members with coordinating a PCP connection following a health Assessment • Meet all quality, customer service, and productivity goals set by the department • Follow internal scripts and processes, track data as requested by Management • Handle additional tasks as requested by Management within expected timeframe • Support business needs by conducting manual outbound calls and handling inbound phone calls • Act as a subject matter expert for peer side-by-side coaching • Accept constructive feedback and respond appropriately • Ability to work overtime and on-call as requested
