• Design, build, and optimize high-throughput RESTful APIs in PHP using the Laravel framework, writing efficient PostgreSQL queries and schema designs to support data-intensive clinical workflows. • Contribute to new feature development and platform improvements within an Agile/Scrum process. • Build and maintain integrations with third-party APIs and internal services, deploying and running services in Docker containers across AWS (or comparable cloud) environments to ensure secure, reliable data exchange. • Write automated tests (unit, integration, and API-level) for PHP/Laravel back-end functionality and critical clinical workflows, and integrate them into CI/CD pipelines using Git/GitHub-based workflows and Docker to enable continuous testing and faster, more reliable releases. • Write and maintain clear technical documentation for troubleshooting guides, fixes, API integrations, and support processes, version-controlled through Git/GitHub for easy team access and updates. • Monitor application health, performance, and error logs using tools such as Rollbar and AWS CloudWatch to proactively identify and address potential issues before they affect patients or providers. • Identify, document, and track software defects, analyzing logs and debugging to pinpoint root causes, and work with the development team within Agile/Scrum sprints to prioritize and ensure timely resolution. • Actively participate in code reviews to uphold coding standards and engineering best practices. • Continuously evaluate and improve our development processes, CI/CD pipelines, and tooling (Docker, monitoring/logging systems, cloud infrastructure) to enhance team efficiency, code quality, and release velocity. • Identify opportunities to formalize and document internal processes, workflows, and troubleshooting playbooks, building a knowledge base that supports faster onboarding and more consistent support.

Role Description We need a senior .NET/C# engineer who has operated real production systems under load — not just built them. Someone who thinks in architecture first, can design for resilience and scale, and knows what it means to keep a highly integrated backend alive across many moving parts. - Core backend services: campaign orchestrators, FSM-based workflow engines, API integrations with telephony (Twilio), voice AI platforms, insurance carriers, and CRMs. - System design and architecture decisions alongside the CTO. - Multi-server deployments, async processing pipelines, event-driven flows. - Frontend integration via Next.js (familiarity needed, not the focus). - Deep .NET / C# expertise. - Strong CS fundamentals: OOP, SOLID, SOA, design patterns. - Experience designing and operating event-driven backend architectures. - Experience running systems on multi-server infrastructure with real traffic — you know what horizontal scaling, fail over, and stateless design actually mean in practice. - Proficiency with parallel and concurrent programming — async/await, thread safety, queues, etc. - Experience integrating many external services/APIs in a single backend — you've dealt with the chaos and know how to tame it. - Ability to design backend architecture from scratch for complex web applications. - Fluent in AI dev tools: Claude Code, Cursor, GitHub Copilot — part of your daily workflow, not occasional use. - Communicates clearly in English; Spanish is a plus.
