Cardiology Patient Navigator

Role Description Coordinates patient care and clinical operations for a U.S.-based cardiology team, with a focus on communication, documentation, and workflow support. Our client is seeking a Cardiology Patient Navigator (Coordinator) to support a busy cardiology care team. The role centers on patient communication, scheduling, care coordination, and clinical documentation. The ideal candidate brings a clinical background, experience in telephonic or remote care settings, and comfort managing a high-volume patient caseload across multiple priorities. Cardiology-specific experience is preferred but not required. Candidates with strong backgrounds in care coordination, care management, med-surg, ICU, cardiac step-down, CCU, or home health will be competitive. What matters most is clinical credibility, strong communication, and the ability to keep patients connected to their care plan. Key Responsibilities - Patient Communication & Scheduling - Manage patient scheduling, reminders, and follow-up workflows in EMR/EHR systems - Conduct telephonic or remote patient outreach to reinforce care instructions and coordinate next steps - Support patients navigating cardiology care pathways, including CHF management protocols - Care Coordination - Track testing schedules, follow-up appointments, and remote patient monitoring workflows - Coordinate with the clinical team to ensure continuity of care across a caseload of 150+ patients - Escalate clinical concerns to the appropriate provider within scope (no diagnosing or prescribing) - Documentation & Compliance - Maintain accurate and timely clinical documentation in EMR/EHR systems - Ensure all patient interactions and records meet HIPAA compliance standards - Support billing documentation and insurance coordination as needed Qualifications - Clinical healthcare background: CNA, MA, RN, or equivalent credential - 3+ years of experience in healthcare, telehealth, or medical office settings - Experience in care coordination or care management, preferably in telephonic or remote settings - B2+ English proficiency | B2+ Spanish proficiency (bilingual required) - Experience using EMR/EHR systems - Strong patient-centered communication skills - Ability to work U.S. business hours (CT) - Strong written and verbal English communication; assessed at screening Preferred - Experience in cardiology, med-surg, ICU, cardiac step-down, CCU, or home health settings - Outpatient cardiology, home health, or telemedicine experience - Remote patient monitoring exposure - Medical billing or prior authorization knowledge - Prior experience working as a contractor or in startup/evolving environments Who Will Succeed - This role is a strong fit for someone who is patient-first, organized under pressure, and genuinely comfortable on the phone. - The right candidate thrives in a high-volume, remote environment where they are managing many moving pieces at once and doing so with warmth and clinical precision. - Friendly, collaborative, and outcomes-focused - Comfortable balancing 150+ patients across multiple priorities simultaneously - Self-directed in a remote setting with evolving processes - Proactive communicator with providers and patients alike - Confident adapting to new systems, tools, and workflows in a growing environment

Responsibilities - Remote Patient Monitoring and Clinical Triage: - Review patient data from Sensorum’s remote monitoring system to identify signs of clinical decompensation. - Assess patients for exacerbation of chronic conditions (e.g., CHF, COPD, diabetes, dehydration, pneumonia). - Conduct timely telephonic assessments with patients and/or caregivers to determine clinical status. - Triage and escalate cases requiring intervention, including coordinating transport to outpatient clinics, urgent care, or the ER as appropriate. - Provide clinical decision-making support and recommendations for next steps in care. - Care Management and Coordination: - Engage patients and families to provide education on managing chronic conditions and early warning signs. - Collaborate with Sensorum’s partners and operations teams to ensure timely follow-up and care delivery. - Document all assessments, clinical findings, and care recommendations accurately in the system. - Patient-Centered Approach: - Act as a trusted clinical resource, ensuring compassionate and proactive patient communication. - Identify opportunities to improve patient outcomes through early detection and intervention. - Support patient empowerment and satisfaction by leveraging technology for engagement and patient education.
