Role Description The Medication Therapy Management (MTM) Pharmacy Technician at PipelineRx (now a part of Evernorth Health Services) supports Remote Pharmacy Services by collecting and documenting accurate patient medication histories, assisting pharmacists, and performing related clinical and administrative tasks. Shift: You will be required to work from 9a – 6p, EST Mon - Fri. Position Summary: The Pharmacy Technician Lead supports the daily operations of the MTM team while also performing MTM technician duties. This role helps coordinate workflow, supports training and onboarding, provides guidance to team members, and works with leadership to maintain quality, productivity, and service goals. Key Responsibilities: - Lead daily MTM team operations while performing Medication Therapy Management technician duties, including patient outreach, appointment scheduling, and documentation. - Support workflow coordination, workload distribution, and team productivity to ensure service and quality goals are met. - Train, onboard, coach, and mentor MTM technicians while serving as a resource for process and operational questions. Qualifications - National Pharmacy Technician Certification required. - 4+ years Pharmacy Technician experience; 3+ year(s) with Medication History Experience in hospitals. - Prescription data entry and project coordination experience. - Strong computer skills. - Experience with Epic, Cerner and/or MediTech. - Ability to work independently or within a team. - Strong communication, interpersonal, and technology skills; emergency availability. Requirements - You will be required to have reliable internet connectivity provided through a wired ethernet connection. A mobile or hot spot environment is not acceptable, and you may need to purchase an Ethernet cord depending on your current setup. - If you will be working at home occasionally or permanently, the internet connection must be obtained through a cable broadband or fiber optic internet service provider with speeds of at least 10Mbps download/5Mbps upload. Responsibilities - Remote Patient Monitoring and Clinical Triage: - Review patient data from Sensorum’s remote monitoring system to identify signs of clinical decompensation. - Assess patients for exacerbation of chronic conditions (e.g., CHF, COPD, diabetes, dehydration, pneumonia). - Conduct timely telephonic assessments with patients and/or caregivers to determine clinical status. - Triage and escalate cases requiring intervention, including coordinating transport to outpatient clinics, urgent care, or the ER as appropriate. - Provide clinical decision-making support and recommendations for next steps in care. - Care Management and Coordination: - Engage patients and families to provide education on managing chronic conditions and early warning signs. - Collaborate with Sensorum’s partners and operations teams to ensure timely follow-up and care delivery. - Document all assessments, clinical findings, and care recommendations accurately in the system. - Patient-Centered Approach: - Act as a trusted clinical resource, ensuring compassionate and proactive patient communication. - Identify opportunities to improve patient outcomes through early detection and intervention. - Support patient empowerment and satisfaction by leveraging technology for engagement and patient education.
