CentraCare
Role Description CentraCare is looking for a qualified Clinical Documentation Nurse to join our team! This role is responsible for concurrent auditing of medical record documentation to evaluate the documentation and utilization review of services to adequately reflect care provided and resources used. - Facilitation of appropriate physician documentation of care to accurately reflect patient severity of illness and risk of mortality. - Proactively solicits clarification from physicians and clinical staff in areas where further specificity is needed in the documentation. - Consults with attending physician when conflicting information appears in the medical record. - Maintains documentation regarding results of audit activity and shares those with staff and management as necessary. - Collects and analyzes data pertinent to documentation findings and outcomes for process improvement. - Supports the concepts and philosophies of nursing as well as sound business practices. - Promotes and facilitates education to providers regarding disease processes. - Communicates with appropriate healthcare team members to promote accurate and complete documentation of each patient encounter. Role Description We are currently recruiting for a detail-oriented, team-focused Medical Writer with a focus in document quality control (QC). In this role, you will support Lexicon across multiple functional areas including: - Clinical Operations - Medical Affairs - Regulatory Affairs - External Affairs - CMC teams Your responsibilities will include: - Collaborating with functional areas to write, review, and QC documents including clinical study reports, protocols, Investigator Brochures, publications, presentations, and other clinical/regulatory documents for clarity, brevity, and consistency with the overall development program and source documents. - Driving the document development process for the functional review team, including using medical writing templates and document management software (e.g., StartingPoint and Veeva Vault). - Synthesizing clinical data, information, references, and other sources with input from SMEs program team members to prepare and present data and document messaging consistent with regulatory requirements, guidelines, and program strategy. - Reviewing and QCing documents. - Creating and managing document timelines. - Attending project-related meetings. - Assisting team members with document locations. - Developing and maintaining a Lexicon Style Guide. - Collaborating with vendors, which may include vendor oversight. - Identifying and appropriately communicating or escalating risks associated with document delivery or other writing-related issues. - Completing project-specific and company training, as assigned.
