RN Navigator
E-Verify Program
• Provides care management support to a panel of patients who require specialty services integrated and in support of their overall plan of care. • Interacts with and supports the specialty care providers, primary care providers and the interdisciplinary care team across the continuum of care. • Assesses, plans, implements, documents, coordinates, monitors, evaluates, and updates the plan of care by collaborating with all members of the health care team to provide evidenced-based care. • Works to establish collaborative processes that promote quality and cost-effective care that optimizes the physical and psychosocial health of patients. • Responsible for supporting decisions that impact health care outcomes, resource allocation and customer experience. • Engages in quality improvement initiatives and program development.
Job Requirements
- Bachelor's degree in nursing
- 2+ years of nursing experience in area of clinical specialty, care coordination or care management
- Licensed Registered Nurse - MN Board of Nursing required
- Licensed Registered Nurse - WI Dept of Safety & Professional Services required if providing care to patients over the phone, through e-visits, virtual visits, or medical messages and the patient is in Wisconsin at the time of the care
- Must meet at least ONE of the requirements below:
- ◦ Current BLS certification from the American Heart Association
- ◦ Current BLS certification from the American Red Cross
- ◦ Allina in-house BLS training (within 30 days of hire)
- Case Manager Certification or Specialty Certification preferred

RN Navigator – CKD
Evergreen NephrologyEmpowering nephrologists to transform kidney care and improve patient outcomes
• The CKD RN Navigator is responsible for providing care coordination, education, and support to patients with chronic kidney disease (CKD) across various stages of the disease. • This role serves as a liaison between patients, families, nephrologists, primary care providers, and the interdisciplinary healthcare team to optimize patient outcomes, promote self-management, and ensure seamless transitions of care. • Leveraging an integrated technology platform, you are complemented by an entire interdisciplinary team including Nurse Practitioners, Nurse Care Managers, Care Coordinators, RN Educator, Dietitians, Pharmacists, Licensed Clinical Social Workers, and Psychiatrists. • You will take the lead on identifying care gaps and navigating the patient through their CKD to ESKD journey in collaboration with a Care Coordinator, RN Educator, and the primary Nephrologist. • You will play an essential role in helping patients achieve their goals through timely and proactive care planning toward a planned outpatient dialysis start with a permanent access or a care pathway of the patient’s choice driven by patient education and care team support.
• Oversight of site level tasks associated with efficient site activation in accordance with ICH/GCP guidelines, local regulations, SOPs and project requirements • Identifying potential research sites that align with the study's criteria and assessing their capabilities and resources • Conduct initial site outreach, i.e., site engagement, collection of Confidentiality Agreement, and feasibility survey • Remote Pre-Study Visits and follow up documentation, including visit preparation, trip report completion, and other documentation as required • Support customization and translation of informed consent forms and patient-facing materials • Collect, track and perform a quality review of all essential documents required for effective and compliant study site activation • Maintenance and ongoing awareness of site regulatory compliance, supporting IRB/Independent Ethics Committee (IEC) annual renewals, PI change-over, and document expirations • Actively participate in team meetings to facilitate study progress and implement action plans for site level risks, underperforming sites, or issues impacting project milestones • Support the CRA team in preparing for site initiation visits (SIVs) • Responsible for submissions to and liaise with applicable IRB/IEC, Third Bodies, and Regulatory Authorities • Perform initial contract and budget negotiations with the sites • Ensure high quality documents are filed in applicable systems in accordance with SOPs and project requirements and that the Trial Master File (TMF) is updated and audit ready at all times • Track the progress of the start-up process and ensure that the research site meets all requirements and deadlines • Perform Case Report Form (CRF) review and data validation against source documentation according to protocol and guidelines as applicable in combination with monitoring for site protocol deviations.
