Job Description

Psychiatric Consultant, Collaborative Care

Cabana

Role Description As Cabana launches its Collaborative Care (CoCM) program, we are seeking a Psychiatric Consultant to join our founding clinical team. This is not a traditional consulting role within an established system — it is an opportunity to help build and define Cabana’s psychiatric consultation model from the ground up. In this remote, contract-based role, you will serve as a core clinical partner supporting Behavioral Health Care Managers (BHCMs) and Primary Care Providers (PCPs) across Louisiana. You will provide evidence-based psychiatric consultation, guide measurement-based care initiatives, and help establish the workflows and clinical standards that will shape Cabana’s integrated behavioral health programs as they scale. This role is ideal for a psychiatrist who thrives at the intersection of clinical excellence, systems-building, and collaborative care innovation. You should be energized by helping primary care teams manage behavioral health conditions effectively while contributing to the design of a new integrated care infrastructure. This is a contract role expecting up to 10 hours a month. What you will do - Structured Case Consultation - Provide systematic psychiatric consultation to Behavioral Health Care Managers (BHCMs) through Structured Case Review (SCR), with emphasis on: - High-risk patients - Patients new to treatment - Individuals not meeting clinical targets - Complex diagnostic presentations - Advise Primary Care Providers (PCPs) regarding: - Evidence-based pharmacologic treatment recommendations - Diagnostic clarification strategies - Lab monitoring and medication management considerations - Document treatment recommendations directly within partner electronic health record systems to support seamless care coordination. - Remain available for occasional urgent consultations with PCPs regarding complex patient presentations, targeting response times within one business day (or one hour for urgent needs). - Population Health & Registry Management - Utilize Cabana’s clinical registry and measurement-based care systems to: - Track patient progress across assigned caseloads - Identify patients requiring treatment escalation or care adjustments - Support population-level behavioral health management initiatives - Participate in weekly caseload review meetings with BHCMs and interdisciplinary care teams. - Review validated outcomes data utilizing tools such as: - PHQ-9 - GAD-7 - Columbia Suicide Severity Rating Scale (C-SSRS) - Support treatment-to-target care planning and symptom remission strategies. - Provider Education & Clinical Collaboration - Serve as a clinical mentor and psychiatric resource for PCPs and BHCMs. - Provide guidance regarding: - Behavioral health treatment planning - Diagnostic conceptualization - Medication management strategies - Management of complex psychiatric presentations - Collaborate closely with interdisciplinary care teams to improve integration of behavioral health services within primary care settings. - Maintain strong communication and collaborative relationships across remote care teams. - Program Development & Workflow Design - Partner with Cabana’s Clinical Director to: - Design and refine psychiatric consultation workflows - Develop escalation pathways and consultation protocols - Establish documentation standards for CoCM implementation - Support development of: - Standard operating procedures (SOPs) - Clinical workflow frameworks - Quality assurance processes - Pilot program launch strategies - Participate in quality review meetings and contribute psychiatric insight to operational and performance improvement discussions. - Perform all other duties, as assigned. Qualifications - MD or DO with an active, unrestricted Louisiana medical license - Board Certified in Psychiatry (ABPN or equivalent) - Minimum of 3+ years of clinical experience, preferably in one or more of the following: - Integrated behavioral health - Primary care behavioral health - Consultation-liaison psychiatry - Collaborative Care (CoCM) environments - Strong knowledge of evidence-based psychiatric medication management for common behavioral health conditions, including: - Anxiety disorders - Depression - PTSD - Behavioral health conditions with medical comorbidities - Experience utilizing measurement-based care tools, including: - PHQ-9 - GAD-7 - C-SSRS - Experience working within electronic health record (EHR) systems, such as Epic, Cerner, or other equivalent EHR platforms. - Strong written communication and clinical documentation skills. - Ability to thrive in a fast-paced, evolving startup environment with changing workflows and priorities. - Must reside within the United States while maintaining active Louisiana medical licensure. Requirements - Prior experience working within the Collaborative Care Model (CoCM) - Familiarity with “treat-to-target” protocols and registry-based population health management - Experience consulting within: - Primary care settings - Federally Qualified Health Centers (FQHCs) - Integrated care environments - Familiarity with CoCM and Behavioral Health Integration billing requirements, including CPT codes: 99492, 99493, 99484 - Comfort managing population-based caseloads rather than traditional appointment-only psychiatric models. - Strong collaboration and interdisciplinary communication skills. - Demonstrated ability to build workflows and contribute to clinical program development initiatives. - High level of adaptability and comfort operating in early-stage program environments. Who Thrives in This Role - You are a builder: who enjoys creating systems and workflows from the ground up rather than simply maintaining existing processes. - You are clinically rigorous: and deeply committed to measurable patient outcomes and evidence-based care. - You are collaborative and communicate openly: with BHCMs, PCPs, leadership teams, and patients. - You are adaptable: and energized by startup environments where flexibility and creative problem-solving are essential. - You are mission-driven: and passionate about expanding access to behavioral health care through integrated solutions. Why Cabana - Mission-driven team focused on transforming access to integrated behavioral health through evidence-based, patient-centered care. - Opportunity to shape the future of psychiatric consultation within Collaborative Care and integrated primary care settings. - Work directly alongside Cabana's Clinical Director and multidisciplinary care team. - Meaningful impact by helping build scalable behavioral health programs for health systems, employers, and military communities nationwide. - Growth opportunities as Cabana expands its integrated behavioral health services and clinical leadership team. - Competitive compensation.

Role Description We are hiring a Senior Consultant, Benefits to lead high-impact client engagements across benefits strategy, plan design, open enrollment, and ongoing benefits administration. This is not a back-office processing role. You will typically enter client environments through a defined project (e.g., open enrollment execution, benefits redesign, vendor consolidation), quickly assess gaps, execute with precision, and then elevate the conversation to more strategic work. Our clients tend to stick with us for repeat engagements. Our best consultants don't just complete the project — they become indispensable. What Success Looks Like - You stabilize and optimize benefits operations quickly and effectively, building immediate client trust. - You are forward-thinking and identify upstream and downstream risks — compliance exposure, rising costs, plan gaps, and scalability issues — before they become problems. - You are able to synthesize complex benefits data and nuance to the business leaders you support, translating operational issues into strategic recommendations that you are able to support leadership teams to action. - You expand our services and our value to clients organically by uncovering additional needs and opportunities. - Clients request to extend your engagement or broaden your role beyond the initial project. Key Responsibilities - Client Delivery & Execution - Support clients in standing up their benefits - either for the first time or for new entities (domestic and international). - Lead open enrollment processes end-to-end, including plan evaluation, employee communications, and enrollment support. - Oversee day-to-day benefits administration across medical, dental, vision, life, disability, and ancillary lines. - Manage broker and carrier relationships, renewals, and vendor negotiations. - Diagnose and resolve benefits issues, coverage gaps, and compliance risks (ACA, ERISA, COBRA, state mandates). - Own benefits transitions, implementations, and platform migrations. - Strategic Advisory - Assess and redesign benefits programs to support company growth (50–500+ employees). - Advise on plan design strategy — contribution modeling, funding approaches, and competitive benchmarking. - Identify cost optimization opportunities across benefits structures without compromising employee experience. - Partner with leadership to align benefits and total rewards with talent strategy and business goals. - Advise on benefits considerations for multi-state and, where applicable, global expansion. - Client Expansion & Influence - Build strong relationships with founders, executives, and functional leaders. - Collaborate with the HR Kickstart team on opportunities to bring in additional specialists (e.g., payroll, comp, recruiting, HRBP support). - Identify additional needs beyond initial scope and contribute to expanding engagements by leaning into broader HR consultant support where appropriate. Qualifications - 7+ years of experience in employee benefits at scaling organizations, including plan design, administration, and open enrollment leadership. - Prior success at setting up benefits programs for new entities both in the US and globally. - Strong knowledge of benefits compliance — ACA, ERISA, COBRA, HIPAA, Section 125, and state-specific mandates. - Demonstrated ability to lead complex benefits redesigns, vendor transitions, or open enrollment overhauls. - Experience managing broker and carrier relationships, including RFPs and renewals. - Comfortable advising on benefits strategy, not just administration, including modeling, benchmarking, and cost containment. - Experience across a range of benefits platforms, HRIS systems and PEO environments common at most scaling companies (Rippling, Gusto, Justworks, Paylocity, Deel, and Remote.com preferred). - Comfortable operating asynchronously in ambiguous, fast-changing client environments. - We are specifically looking for consultants running an established consulting business or previous employment at a consulting firm. Due to the nature of the work, we are not a match for individuals who are trying out consulting for the first time or are currently committed to a W2 role. Engagement Details - Contract (1099) - Remote from the US - Typically 5–25 hours/week (varies by client and project) - Must operate through your own business entity and carry liability insurance (if you don’t already have liability insurance, you must be willing to obtain it). - Payrate: $90 - $150/hr (varies by client and project)
