• Coordinating prior authorizations, insurance verifications, medication approvals, and medical service approvals • Working closely with providers, pharmacies, insurance companies, and internal clinical or billing teams to obtain required approvals • Resolving authorization issues, and maintaining complete documentation • Frequent computer use, phone communication, insurance portal access, and coordination with multiple departments • This position typically reports to their Regional Manager, DOD Operations Manager, or Project Manager

• The POD Pharmacist helps develop and implement Monogram’s medication therapy management (MTM) program • Perform timely written member medication reconciliations and drug therapy action plans • Conduct telephonic consultations with medical providers when medication changes appear necessary • Work collaboratively with clinical care manager operations personnel to integrate medication management • Drive meaningful medication interventions that result in medical providers’ and members’ adhering to evidence-based medication guidelines • Review member care reports, clinical notes, and other medical records for medication action plan development • Serve as a pharmacist resource for Monogram field clinicians regarding medication use and Monogram MTM resources • Assist with the delivery of MTM training sessions aimed at building a standardized and effective MTM program for Monogram clinical staff • Monitor important medication availability and use trends for populations under Monogram’s care management • Participate in external and internal interdisciplinary care team case rounds or meetings and produce MTM related documentation

• Review and render Part D coverage determination and appeal requests. • Conduct comprehensive medication reviews via telephone for members enrolled in the MTM program. • Participate in pharmacy quality improvement projects aligned with Medicare Star Ratings. • Maintain appropriate records of all clinical reviews, coverage determinations, and consultations in accordance with CMS timelines.
