Job Requirements

  • Active LCSW license in Pennsylvania preferred, but not required.
  • 2+ years of experience as an independently Licensed Clinical Social Worker (LCSW)
  • Full-time availability (40 hours per week) during EST hours.
  • Experience & willingness to treat patients ages 3+ required.
  • Successful completion of the ASWB examination for independent clinical licensure.
  • Must reside in the U.S. or a U.S. territory.
  • Required Clinical Skills: 2+ years of experience working with children and adolescents post-independent licensure
  • Experience treating Anxiety, Depression, ADHD, Trauma, and Disruptive Behavior Disorders
  • Experience in diagnosing ADHD across developmental stages, with familiarity in standardized assessment tools and collateral gathering.
  • Proficiency in evidence-based behavioral treatments for ADHD, including parent management training, organizational skills training, and time-management interventions.
  • Ability to coach caregivers and coordinate care with schools and prescribing providers.
  • Ability to complete suicide risk assessments and safety plans.
  • Preferred (not required): Familiarity with Acceptance and Commitment Therapy (ACT) and Dialectical Behavior Therapy (DBT) principles, and the ability to integrate relevant skills (e.g., mindfulness, distress tolerance, values-based activation) into treatment when appropriate, experience working in an integrated care setting.

Role Description As a Discharge Care Manager at UnitedHealth Group, you will be responsible for implementing day-to-day telephonic case management interventions for identified high risk members. This means you will be tasked with assessing and interpreting member needs and identifying solutions that will help our members live healthier lives. The Discharge Care Manager (DCM) will coordinate and document the discharge plan in collaboration with other key clinical care team members. The DCM will also follow the member while in the acute inpatient setting. If you are located in Missouri, you will have the flexibility to work remotely* as you take on some tough challenges.
