RPM Healthcare
• Serve as the primary contact for patients on the RPM365 platform. • Assist with device setup and app connectivity. • Engage with patients to ensure adherence to care plans. • Schedule monthly check-ins and monitor patient data. • Educate patients through shared resources. • Notify healthcare providers of critical alerts. • Attend virtual team meetings for updates.
Job Requirements
- Active nursing license that is a compact/multistate.
- Knowledge of chronic conditions and related monitoring devices (weight management a plus).
- Proficiency with technology, computer platforms, and smartphones.
- Strong verbal/written communication skills.
- Comfortable with patient outreach and engagement.
- Highly organized, detail-oriented, and team-focused.
