Role Description We are seeking a highly skilled Registered Dietitian who is passionate about helping clients make sustainable changes through personalized, evidence-based nutrition care. This is a fully remote, 1099 contractor role. • Coordinate and manage patient care and service delivery in accordance with LHCSA regulations and individualized care plans • Serve as the primary point of contact for patients, providers, and home health aides, ensuring clear and timely communication • Schedule, assign, and maintain appropriate staffing coverage, including managing callouts, emergencies, and shift changes • Match aides to patients based on skills, availability, and patient needs to ensure quality care • Monitor and enforce compliance with Electronic Visit Verification (EVV) requirements, including proper clock-in/clock-out practices • Address missed visits, incomplete shifts, and timekeeping discrepancies while educating aides on compliance standards • Ensure all services are delivered within authorized hours and in alignment with regulatory requirements • Respond to and resolve patient, family, and aide concerns in a timely and professional manner • Escalate clinical, operational, or compliance-related issues as appropriate • Maintain accurate and timely documentation of all interactions, incidents, and updates in the system • Track and report service gaps, compliance issues, and performance metrics • Ensure records meet LHCSA, Department of Health (DOH), and internal audit standards

• Build and manage a pipeline of commercial and industrial prospects. • Develop relationships with business owners, property managers, operators, executives, and other decision-makers. • Identify opportunities to help clients reduce costs, manage energy risk, improve efficiency, and plan for future energy needs. • Present tailored energy solutions using Navigate Power and Verde Solutions’ supplier network, tools, and support resources. • Grow long-term client relationships through consultative selling, referrals, networking, and consistent follow-up. • Stay current on energy procurement, renewables, battery storage, EV charging, and other commercial energy solutions.

• Delivering inclusive, expert, virtual care through scheduled synchronous consultations through our virtual care platform • Leading comprehensive assessments for PrEP, DoxyPEP, STI prevention/treatment, and HIV-related care, including: health history, risk assessment, and psychosocial evaluation • Supporting patients with affirming education on HIV/STI prevention, risk reduction, medication adherence, and testing • Providing timely asynchronous care, including ordering labs and diagnostics, interpreting results, prescribing medication, documentation and completing clinical follow-up • Maintaining accurate, detailed records in the Electronic Medical Record (EMR), using virtual tools to chart, prescribe, communicate, and support seamless continuity of care • Collaborating with and acting as a resource for our team of Registered Nurses and support team members, including managing clinical escalations for complex cases when required
